MARK’S SCHOOL COUNCIL HOT LUNCH PROGRAM SESSION #2 (2012-2013) MARSHALL’S PASTA We will be continuing with our hot lunch program with Marshall’s Pasta, so we need to fill out a hot lunch order form. Our hot lunch day will continue to be every WEDNESDAY and session 2 will run from February to June 2013. Please fill in your hot lunch order form using the on-line form. DIRECTIONS: NOTE: Once you enter the students name you are entering a form in the system so please finish the complete form. 1. Using your computer, go to the website www.marshallspastamill.com 2. Click on the tab: ” Hot Lunch On-line”. 3. Enter the school’s code smk1 and hit “go”. 4. Select the student’s teacher. 5. Enter the student’s name, phone number and parents e-mail address. (E-mail address is optional but may be used by the hot lunch coordinator if she has any questions) 6. Fill out the form. On each hot lunch date you must check choice of food, whether you want an extra serving and cookie. If you do not want to order a hot lunch on a specific date, you must put an “X” in the box. This indicates that the item is NOT ORDERED. 7. Once the form is complete, print off a copy for your records. Payment is to be handed into the students’ teacher with a copy of printed form or a written copy stating student name and teacher. Parmesan cheese is provided. We ask you to take the time to carefully complete the on-line order form. It must be sent in online and returned to teachers by FRIDAY JANUARY 25, 2013. You do not have to order every week, it can be bi weekly or once a month, but the order and the total amount must be in by January 25th, 2013.
