participate in the Lady Hawk Volleyball Camp. I understand that by its nature,
participation in athletics include a risk of injury. I further understand that I will be
responsible for the payment of any and all costs resulting from any treatment
rendered. I hereby waive any claim that I might have against the camp staff,
Hillgrove Volleyball Booster Club, or Hillgrove High School for any and all liability
for injuries incurred while participating in camp activities.
