Introduction
The Trust Integrated Governance and Risk Management Framework defines how risks to patients,
staff, the public and the organisation will be assessed, mitigated and managed. The administration of
this process is through an electronic operating system called Datix, which enables the recording of
risks on risk registers that are defined by organisational structure- for example there is a risk register
for each Care Group, department or team within the Trust.
Process
The policy document defines the process of risk assessment as:
 Defining the hazard
 Estimating the impact or consequence should the hazard occur
 Estimating the likelihood of the hazard occurring
The impact and likelihood are then rated according to the following matrix

Refer to the Trust Governance and Risk Management Framework including Risk Assessment and Risk Procedure (available on the Trust Intranet)
The framework also gives guidance as to what would constitute a catastrophic impact versus a
moderate impact and how to assess the likelihood.
All risk registers are held together in Datix as a corporate risk register. The Trust Integrated
Governance and Risk Management Framework states that risks that are assessed as 15 or above
should be reported to the Trust Board for review on a regular basis. Risks that are currently held on
the corporate risk register that are 15 or above are shown in appendix 1.
It is recommended that the Board note this report.
