The administration of this process is through an electronic operating system called Datix, which enables the recording of risks on risk registers that are defined by organisational structure- for example there is a risk register for each Care Group, department or team within the Trust. All risk registers are held together in Datix as a corporate risk register. The Trust Integrated Governance and Risk Management Framework states that risks that are assessed as 15 or above should be reported to the Trust Board for review on a regular basis. Risks that are currently held on the corporate risk register that are 15 or above are shown in appendix 1. It is recommended that the Board note this report. Current use of the Outlier Policy identifies patients from GPAU/MAU/ED only. This has resulted in elderly, confused patients requiring 'all care' being moved to surgical wards, increasing LOS, reducing bed stock to manage surgical activity and placing at risk patient safety and quality of care. Clinical feedback regarding the suitability of the suture for some specialities and procedures has identified risk in failure of Covidien sutures which may compromise patient safety.
