PRIVATE AND CONFIDENTIAL <Student Name> <Address 1> <Address 2> <Address 3> <Address 4> Phone: Email: Date Dear, Alleged Academic Misconduct – Referral to School Academic Misconduct Panel Module: xxxx Exam/Assignment/Coursework: xxxx I am writing to advise you that the above assessment has been referred under the Academic Misconduct Procedure for an alleged use of unfair means (academic misconduct). It is my role as the Head of School nominee to take a first view as to whether there is a case to answer. After careful consideration of the evidence, my conclusion is that academic misconduct may have occurred and that it requires further consideration. The matter has therefore been referred to the School’s Academic Misconduct Panel (AMP). I enclose a copy of the relevant documentation received for information. The membership of the AMP will be: 1. a Chair who is an external member of staff from outside the School; 2. the Associate Head Academic (or equivalent) from within the School; 3. a student representative nominated by the Students’ Union. A copy of the University Academic Misconduct Procedure is attached and may also be found at: http://www.governance.salford.ac.uk/page/student_policies A meeting of the Panel has been arranged for xxxx and will be held in xxxx. You are invited to attend the meeting. Please arrive no later than xxxx. You may be accompanied to the meeting by a friend or representative. If you wish to seek advice from the Students’ Union regarding this matter, you should contact the Student Union Advice Centre on advicecentre-ussu@salford.ac.uk or 0161 351 5400. Information on representation can be found in sections 3.5 and 3.6 of the Academic Misconduct Procedure. You may submit written evidence to the Panel by no later than xxxx, at least 5 working days before the meeting. Written evidence must be sent to xxxx. I would be grateful if you could confirm attendance at the meeting. You are strongly advised to attend the Panel hearing. In the event that you are unable to attend please contact xxxx Panel Secretary xxxx@salford.ac.uk at the earliest opportunity. Please note an Academic Misconduct Panel will normally proceed without the student in attendance unless there is prior agreement to postpone the case. Normally evidence should be supplied to support the mitigation (e.g. sickness absence). Should you attend the hearing, you should declare at the start anything which you consider may impair your performance (e.g. illness, medication). Please note that Personal Mitigating Circumstances evidence cannot be considered by the Panel in mitigation against a case of Academic Misconduct, such matters are dealt with at School level by the PMC Panel, with documentation provided to the same at the appropriate point in time (Academic Misconduct Procedure paragraph 3.8.1 and Appendix C6.1). If you have any queries regarding the content of this letter, please do not hesitate to contact xxxx.
