Initial Referral to Special Education Procedures
1. ____ Parent Consent for Evaluation for Special Education Services must be printed from GOIEP when
referral may be recommended.
a. The RTI Coordinator or ISC must generate this form from GOIEP
b. The form may be printed prior to the meeting or during the meeting
c. Parent Request – if accepted by committee review team, hold a meeting to obtain consent and
start interventions. Contact Maria Carroll asap for testing. Send Parent Request Packet to Julie
Schexnayder, SPED Werz. Keep a copy of the parent request forms and interventions in the school
referral file to continue with these procedures.
Instructions for Assigning a Case Manager and Referral Reason in GOIEP for Initial Referrals:
1. Log in to Infinite Campus (IC), Click SLDS on the left menu in IC, Click IEP on the top menu once in SLDS
2. Click Advanced Search next to the search option
3. Enter the student’s First and Last Name and click look up student
4. Click the Person Icon next to the student’s name to assign a case manager if not assigned yet
5. Enter the first and last name of the referring person – typically RTI Chair and Click Search
6. Click Assign next to the person’s name. A pop up box should open to enter the referral reason
7. Click the circle next to the correct referral reason in the pop up box and Click Save Referral Reason (once
selected this reason cannot be changed)
8. The screen will be black after you click save – click the screen
9. To begin the Consent for Evaluation follow the instructions below for Printing the Consent
10. If finished, Click Logout on the top right corner
Instructions for PRINTING the Consent for Evaluation for Special Education Services from GOIEP:
1. Log in to Infinite Campus (IC), Click SLDS on the left menu in IC, Click IEP on the top menu once in SLDS
2. Click Advanced search again – this is important to do to go back to your student if continuing from the
instructions above.
3. Enter the student’s first and last name and Click look up student
4. Click on the student’s Name NOT the icon.
5. Click the blue Timelines tab at the TOP
6. Click the Override Timeline Dates (blue link on the right side of the screen)
7. Click Submit with No Changes
8. Click Timelines tab again at the top
9. Click the Start Consent to Evaluate link (blue link on the right side of the screen)
10. Enter the person Referred by or enter SST or Parent Request
11. Enter the date the form will be sent to the parent (cannot be a future date right now so generate day of
the meeting)
12. Click Save
13. Click Print Parent Consent for Evaluation at top right corner to print the form. It will open in adobe.
14. Print the form once it opens. Close the form after printing
15. Click Close on top right of the pop up box
16. Once you have signed consent, go to step 3 below. If you already have Consent signed and want to
continue with these instructions go to step 3 below and start with instruction number 6.
17. If finished, Click Log Out to log out of GOIEP
2. ____ Obtain Parent Consent for Evaluation for Special Education Services
a. RTI chairs should obtain this consent at the SST meeting if a referral to special education is being
made so there is no perceived delay.
b. Be sure to give a copy of Special Education Parent Rights and explain those rights.

3. ____ Enter the date Parent Consent for Evaluation for Special Education Services was signed by the
parent and the date received by the school in GOIEP.
Instructions on ENTERING Parent Consent for Evaluation for Special Education Services DATES in GOIEP:
1. Log in to IC; click SLDS, Click IEP
2. Click Advanced Search
3. Enter the first and last name and click look up student
4. Click the student’s name not the icon.
5. Click the blue Timelines tab at the top
6. Click the Consent to Evaluate In Progress (blue link on the right side of the screen)
7. Enter Yes or No, Enter Parent Name, Enter Date Received and Date Signed
8. Click Save Parent Consent after all above info has been entered
9. To continue with these instructions and enter Case History and Progress Monitoring Data go to step 4
below and start with instruction number 6.
10. If finished, Click Logout
4. ____ Enter the Case History and Progress Monitoring data in the eligibility report in GOIEP and click
validate to ensure all data has been entered correctly.
Instructions to ENTER Case History and Progress Monitoring data in GOEIP:
1. Log in to IC; click SLDS, Click IEP
2. Click Advanced Search
3. Enter the first and last name and click look up student
4. Click the student’s name NOT the icon
5. Click the blue Timelines tab at the top
6. Click Start Initial Eligibility (blue link at the right side of the screen)
7. Click the blue Eligibility tab at the top
8. Click View/Update (blue button)
9. You are in the eligibility report at this time. You are in the Case History – gray tab
10. Answer questions 1.1 through 1.12 in Case History
11. Click Save Case History
12. Click Progress Monitoring (gray tab at the top)
13. Answer question 2.1 yes or no – the answer should be no unless a Sped Program Specialist has been
involved with this referral. These are rare cases.
o If yes, justify the decision and click save progress monitoring and stop here
o If no, continue with instructions
14. Answer question 2.2 – Choose the area of difficulty and click add details. You must do this for each area
of difficulty where RTI has been implemented and data collected. At least one area must have two
interventions for that area.
o Indicate the Intervention, beginning date, end date or check in progress, and frequency of the
intervention
o Enter the Baseline Performance Data and Date and Summary
o Enter the Intervention Date (start date of the intervention), Intervention Data (dates and data
points), and Summary of the data
o Click Update (blue button)
o Click Close (blue button)
15. Click Add Details button to add a second intervention for that area
16. Click Area of Difficulty and change area to add another area of difficulty and interventions for that area.
17. When all areas and interventions have been entered:
18. Click Save Progress Monitoring (blue button) –after all areas of difficulty and intervention data have been
included

19. Click Logout button on top right
5. ____ RTI Coordinator completes Initial Referral to Special Ed Checklist form
6. ____ RTI Coordinator submits initial referral file/SST folder to ISC with Initial Referral to Sped Checklist
a. The SST file with all appropriate forms will be sent to the ISC
b. Once consent for evaluation is signed, the 60 day timeline begins. The file must be forwarded
within 3 days to the ISC.
c. RTI continues during the referral and evaluation. Updated RTI data will need to be presented at the
next meeting.
7. ____ ISC reviews the file
a. ISC checks the file to ensure all appropriate documents are included – Referral to Special Ed Form,
Parent Consent, Passed Hearing/Vision, Psycho-educational Report, SLP Evaluation Report if
needed, etc.
b. ISC checks file in GOIEP to ensure Case History and Progress Monitoring data has been completed
8. ____ ISC completes Initial Referral to Special Education Checklist and submits required documents to
SPED office
a. Submit the checklist and Parent Consent for Evaluation of Special Education Services and any other
required documents per the checklist to Werz Central Office to Julie Schexnayder.
9. ____ SPED records clerk logs in the initial referral
a. SPED records clerk logs the initial referral and indicates due dates on step 2 of the Initial Referral to
Special Education Checklist.
b. Copy of the checklist is given to the Sped Program/Support Specialist and the original is sent back
to the school
10. ____ ISC completes step 3, 4, and 5 of Initial Referral to Special Education Checklist
a. Step 3 - Complete Draft Eligibility in GOIEP
i. ISC gathers work samples, observations, statement of status checklists, and requests
additional testing, medical forms, or rating scale forms if needed and enters appropriate
data in GOIEP
b. Step 4 - Hold Eligibility Meeting
i. If Eligible, IEP meeting must be held within 30 days of the eligibility meeting date.
1. Parental Consent for Placement must be signed AFTER the IEP has been developed.
a. If Parent refuses to sign consent or checks No, the student returns to regular
education.
2. The SST file becomes the special education file and is submitted to CO with
completed Initial Referral to Special Education Referral Checklist
ii. If NOT Eligible, the SST file remains at the school. The completed Initial Referral to Special
Education Referral Checklist is completed and submitted to CO.
11. ____ Submit Change in Status Form and completed file to CO
a. ISC submits change in status form and completed file to Werz CO Julie Schexnayder
b. Program/Support Specialist complete eligibility report and finalize and submit file to Julie
Schexnayder along with Change in Status form to ensure file is complete and submitted
