STEP students planning to pursue additional endorsements must complete the questions below. Please
sign, date and return the form to the Office of Student Services or email it to mnielsen@uw.eud by
January 31, 2014.

I plan to incorporate additional endorsement requirements into my MIT program and will
not file for graduation in Winter 2014. By delaying graduation I acknowledge (please initial
each statement):
_____ I must apply for graduation by the deadline set by the Graduate School in the quarter I

_____ The Graduate School requires that I am registered for at least one credit in the quarter I
graduate.
_____ My MIT degree will not be issued until I graduate.

credits count toward my masters degree. They will not appear as additional credits on
