Community Therapy Service Summary Points to Remember At the Community Therapy Service clinicians from a number of different disciplines work with you to help you achieve your rehabilitation goals. What is the Community Therapy Service? The Community Therapy Service (CTS) helps people who experience difficulty doing day-to-day tasks work towards the goals that are important to them. We work with people who experience various conditions such as: Age related changes Musculoskeletal conditions Rheumatologic conditions Neurological conditions Chronic Pain Amputation Vestibular conditions Cardiac Conditions What will happen at the Community Therapy Service? The CTS therapists will work with you and your carer/s to help better manage your condition. You may do exercises or activities with the therapists or they may recommend a program for you to complete at home. Each CTS program is different and is designed to suit your needs. As such, you may receive your therapy as an individual or as part of a group. The therapists may see you in your own home, at RMH Royal Park Campus or a combination of both. The length of time you will be involved with CTS will be established with you, taking into account your goals and capabilities. Who will I see? We have people from the following disciplines in our team: Clinical psychology Physiotherapy Dietetics Prosthetics and Orthotics Exercise Physiology Recreational Therapy Doctors who specialise in aged care Doctors who specialise in rehabilitation Neuropsychology Social Work Nursing Speech Therapy Occupational Therapy Who is eligible? Adults with relevant rehabilitation goals who live in the local council areas of Melbourne, Moreland and Moonee Valley. How do I arrange an assessment? Referrals can be made via the Direct Access Unit (see details at bottom of page). Appointments are usually made by referral from your family doctor or specialist however enquiries are welcome from anyone. Page 1 of 3 How much does it cost? There is no cost for therapy. What if I am not able to make it to an appointment? If you are unable to attend an appointment please contact CTS Reception 8387 2175 at least 24 hours prior to your appointment to allow the therapist to reschedule another client for that time. Although CTS has a waitlist, every effort is made to ensure that all our clients receive timely therapy to maximise their rehabilitation. If less than 24 hours notice is provided this will be recorded as nonattendance. Missing two appointments may result in a discharge from the service. How do I get there? CTS is held at: Graeme Larkin’s Rehabilitation- Building 26 RMH Royal Park Campus 34-54 Poplar Road Parkville 3052 Taxis can drop you off at the front of the Graeme Larkin’s Building, RMH Royal Park Campus. Car Parking is available at a cost of $7 payable on exit with coins only. Disabled persons’ parking is available close to the Graeme Larkin’s Building. Enter via Poplar Rd via Gate Four. Transport Assistance You are expected to make your own way to and from appointments. If there are any concerns regarding transport please discuss these with the therapist during your initial assessment. Please be aware that if you choose to access our limited transport service there are significant waiting periods and you will be required to pay $10 per trip which needs to be paid on the day at CTS reception.
