Tim Gullikson
Tim Gullikson was named the ATP Newcomer of the Year in
1977. He went on to capture four singles titles in his
professional career and reached the finals in 11 singles events.
He had a career-best year-end world ranking of number 18 in
1978 and was ranked among the top 50 in the world in six of
seven years between 1977 and 1983. Tim teamed with his
brother Tom to form the winningest brother doubles
combination in the open era winning ten doubles titles together.
They reached the Wimbledon Men’s Doubles Semifinals in
1982 and the Men’s Doubles Final in 1983. They also reached
the US Open Men’s Doubles Semifinals in 1982. In all, Tim
won 16 doubles titles and reached the finals in 13 others. He
won five over-35 titles in 1991 including Wimbledon. He also
won 21 over-35 doubles titles. Tim served as Pete Sampras’
coach helping Sampras to a number one world ranking. Tim
passed away in May of 1996 from brain cancer. He was
posthumously inducted into the USTA/Midwest Section Hall of
Fame in 1997.

Tom Gullikson
During his professional playing career, Tom Gullikson reached at least
the third round of all four Grand Slam Championships, the doubles
semifinals in three of the four Grand Slams, and he teamed with
Manuela Maleeva to win the 1984 US Open Mixed Doubles
Championship. Tom achieved a career high singles ranking of number
38 and a career high doubles ranking of number nine. He finished
among the world’s top 75 in eight of nine years between 1977 and
1985. He won 12 career doubles titles and one career singles title. Tom
was named the USTA Director of Coaching in 1997. He served as the
men’s coach for the 1996 US Olympic Team in Atlanta where he
coached Andre Agassi to a gold medal. In 1993 he was named the 36th
US Davis Cup captain. He was named the United States Olympic
Committee Elite Coach of the Year for Tennis in 1996. He also coached
the men’s tennis team at Northern Illinois in 1974. He, too, was
inducted into the USTA/Midwest Section Hall of Fame in 1997.

Rick Van Horn
Rick Van Horn was a passionate USTA/Midwest Section volunteer. Rick came to the Indianapolis area to
serve as the Director of the Indianapolis Tennis Center in April 1997, from Portland, Oregon, where he had
been serving as the Director of Player Development for the USTA/Pacific Northwest.
Before moving to the USTA/Midwest Section, Rick had served in many volunteer roles for the
USTA/Pacific Northwest. He was elected President of the USTA/Pacific Northwest and served in that
capacity for five years from 1990-1994.
As a USTA national volunteer, Rick served on several committees including Nominating, Sectional
Presidents, USTA Leagues, Junior Competition, Adult Recreation, Tennis Rules, and Adult Competition.
In the USTA/Midwest Section, he served as Chairman of the Junior Competition Committee from 2002 to
2007, and Chairman of the District Presidents' Committee in 2004 and 2005. He was Chairman of the Junior
Davis Cup Committee from 2000 to 2001, and served on the Section Championship Committee in 1999.
At the local and District level, Rick was a founding member of the Indianapolis Community Tennis
Association and served as its Treasurer for two years. He served a two-year term as President of the Central
Indiana Tennis Association from 2005-2006.

Richard Bradley’s volunteer service began at the grassroots level in his community and extended through
different levels at the USTA with an emphasis on promoting and growing junior tennis. Bradley believed
that tennis could have a powerful impact in local communities and in the lives of children.
At the USTA level, Bradley volunteered for the USTA/Midwest Section for 13 years, serving as chairperson
of the Section’s Junior Competition Committee and Multicultural Participation Committee. He later served
on the USTA/Midwest Section Board of Directors for four years, and was eventually elected as a Delegateat-Large on the Executive Committee.
At the local level, Bradley was very active in the Chicago District Tennis Association, serving in volunteer
and leadership roles for approximately 20 years, which included time on the Chicago District Board of
Directors. He was elected President of the Chicago Prairie Tennis Club in the early 1980s serving for several
years.
Bradley was elected President of the American Tennis Association in 1994 and served for approximately six
years. He also was President of the Midwestern Tennis Association (MID-TAC), the Midwest Section of the
American Tennis Association, for approximately eight years.
Through his involvement with the Greater Chicago Community Tennis Association, Bradley became a
mentor to former professional tennis player Katrina Adams during her junior career growing up in Chicago.

Tim & Tom Gullikson Scholarship
The Tim & Tom Gullikson USTA/Midwest Section
Scholarship was established by the Midwest Youth
Tennis & Education Foundation in January 1998 to
recognize Tim & Tom Gullikson, and to reward young
people with funding to advance their educational and
tennis interests. Each year, a $2,500 Tim & Tom
Gullikson USTA/Midwest Section Scholarship will be
presented to one female and one male junior tennis
player from the USTA/Midwest Section.
The Tim & Tom Gullikson USTA/Midwest Section
Scholarship is designed to provide assistance to those
who have demonstrated financial need and have shown
ability, aptitude, and promise both as students and as
tennis players.
Requirements
Each applicant must:
 Be a resident of the USTA/Midwest Section;
 Be a current USTA member;
 Be a high school senior;
 Plan to attend an accredited college or university;
 Submit a completed application packet.

ability, aptitude, and promise both as students and as
tennis players.

 Plan to attend an accredited college or university;

former tennis professional Katrina Adams in 2014. Each
year, a $2,500 Richard Bradley Memorial
USTA/Midwest Section Scholarship will be presented to
one junior tennis player from the USTA/Midwest
Section.
The scholarship is designed to provide assistance to
those with a passion to work in the tennis industry, have
demonstrated financial need, and have shown ability and
aptitude as students and as tennis players.

The Hooked on Tennis USTA/Midwest Section
Scholarship was established by the Midwest Youth
Tennis & Education Foundation in January 2011 in
collaboration with a local tennis organization in
Northern Illinois, and to reward young people with
funding to advance their educational and tennis
interests. Each year, a $1,000 Hooked on Tennis
USTA/Midwest Section Scholarship will be presented
to one female and one male junior tennis player from
the USTA/Midwest Section.
Each applicant must:
 Be a resident of the USTA/Midwest Section;
 Be a current USTA member;
 Be an African American high school senior;
 Be an A or B academic student;
 Plan to attend an accredited college or university;
 Play tennis at one of the following levels (USTA,
Midwest, District or locally on high school team)
and/or participate in program such as JTT or NJTL;
 Volunteer for tennis in the local community;
 Submit a completed application packet.

Bradley Memorial Scholarships
Please note: If the award of the Tim & Tom
Gullikson USTA/Midwest Section Scholarship, Rick
Van Horn USTA/Midwest Section Scholarship,
Hooked on Tennis, or Richard Bradley Memorial
USTA/Midwest Section Scholarship to an applicant
would result in the reduction of the amount of an
academic or athletic scholarship(s) awarded to the
applicant by a college or university, then the
applicant is ineligible to receive this scholarship.
Applicants must verify that the receipt of the Tim &
Tom Gullikson, Rick Van Horn, Hooked on Tennis,
or Richard Bradley Memorial USTA/Midwest
Scholarship will not result in the reduction of any
academic or athletic scholarship from their college
or university.
Midwest Youth Tennis & Education Foundation
The Midwest Youth Tennis & Education Foundation is
dedicated to supporting youth tennis programs and
activities throughout the USTA/Midwest Section. As
the charitable and philanthropic entity of the
USTA/Midwest Section, its mission is to support youth
by providing opportunities that develop life skills
through tennis.
The Midwest Youth Tennis & Education Foundation
places emphasis on programs and activities that
provide youth with a positive, fun and disciplined
environment to learn the game of tennis. The
Foundation supports community tennis programs for
youth of all ages, skills and backgrounds. Grant
opportunities for individuals are also available.
The USTA/Midwest Section
The Indianapolis-based USTA/Midwest Section
exceeds more than 82,000 individual members and
1,000 organization members. It is the second largest
section of the United States Tennis Association
(USTA), the national governing body for tennis in the
U.S. The USTA/Midwest Section is comprised of 13
Districts in the following states: Illinois, Indiana,
Michigan, Ohio and Wisconsin, as well as designated
counties in West Virginia and Kentucky. The mission
of the USTA is to develop and promote the growth of
tennis. The mission of the USTA/Midwest Section is
to promote, develop and service the game of tennis.
Selection Committee
A committee appointed by the President of the Midwest
Youth Tennis & Education Foundation evaluates the
applications for the Tim & Tom Gullikson, Rick Van
Horn, Hooked on Tennis, and Richard Bradley Memorial
Scholarships. This committee includes members of the
Midwest Youth Tennis & Education Foundation Board
of Directors, and will recommend the final candidates for
approval by the entire Board. The selection will be made
without regard to the applicant’s race, color, ethnic
origin, religious belief or physical handicap.
The recipients of the scholarships are recognized
annually via a video presentation at the USTA/Midwest
Section Spring Meeting, and also contacted by the
President of the Midwest Youth Tennis & Education
Foundation. Each recipient of the Tim & Tom Gullikson,
Rick Van Horn, Hooked on Tennis, and Richard Bradley
Memorial Scholarship receive a certificate
acknowledging the year in which the scholarship was
received.
Application Deadline
All completed applications must be received in the
Midwest Youth Tennis & Education Foundation
office by March 1, 2015. Applications must be
submitted on forms identified by the year in which
the awards will be made.

to apply for the Hooked on Tennis Scholarship.

One $2,500 Richard Bradley Memorial USTA/Midwest Section Scholarship will be presented to a recipient. African
American high school seniors who plan to attend accredited colleges or universities, who are current members of the
USTA, residents of the USTA/Midwest Section, are an A or B academic student, play tennis at USTA, Midwest District or
locally on high school team and/or participate in a program such as JTT or NJTL, and volunteer in tennis in their local
community are urged to apply for the Van Horn Scholarship.
All of these scholarships are designed to provide assistance to those who have demonstrated financial need and have shown
ability, aptitude, and promise as students and as tennis players.
Your candid evaluation of the applicant will be of assistance to the selection committee. Please complete both sides of this form,
and if you wish, attach a letter on behalf of the applicant. This form should be signed and sealed in an envelope and then returned
