Application for approval is required only for the following works (as shown in a) that are
within the corridor distance of concern (as shown in b).

I confirm that:
(1) I am the qualified person for the project/works. I hereby apply for approval
for carrying out the above-mentioned works which are likely to affect the public
sewerage system within or in the vicinity of the development /work site.

(2) All the existing and proposed buildings/structures, piling works including
temporary works for excavation and all the proposed & existing sewers are
shown in the plans submitted with this application.

(3) The exact position and levels of all the existing sewers as indicated in the
original copy of SIP attached to this submission and as shown in the submitted
plans have been checked and verified on site.

(4) The proposed development/works *do not involve/involve building/ structure
directly over or above any sewers or encroach on any minimum clearance
distance between structure and sewer as stipulated in the Code of Practice on
Sewerage and Sanitary Works or published in the PUB Internet Website.

I also declare that I will take every practical measure to ensure that the
proposed development/works will not disrupt or affect any existing public
sewers. I will not commence any temporary works/piling works/building works
within the required setback for the existing sewer until a written clearance from
Water Reclamation (Network) Department, PUB has been obtained.

This application is accompanied by:
(a) For all sewers within and in the vicinity of the development site as per
PUB consultation requirements
All the below documents as attached:
(1) Sewerage Interpretation Plan (SIP) showing the existing sewers near or
within the site.
(2) Pre-condition survey plan endorsed by a Registered Surveyor showing the
position, sizes, top and invert levels of the sewers and manholes (horizontal
and vertical) in relation to the existing buildings/structures
(including wall, drains, etc) and boundary lines.
(3) Layout plan endorsed by QP showing the proposed buildings/structures
(including retaining and boundary wall and footings, drains etc) overlaid on the
survey plan mentioned above. The plan shall indicate the minimum setback for
the existing sewers.
(4) Piling layout plan endorsed by QP showing details of the proposed piling
works including the horizontal and vertical clearances between the piles and
the existing sewers.
(5) 1 set of Pre-Condition CCTV inspection survey reports together with CCTV
video in accordance with PUB (WRN)'s CCTV inspection requirements posted
on PUB's Internet Website. The CCTV reports and DVD discs are submitted in
an envelope labeled "PRE-CONDITION CCTV INSPECTION REPORT" &
"Project Ref No.". The CCTV inspection survey was carried out for all existing
sewers and manholes within and in the vicinity of the development site as per
PUB (WRN) CCTV requirements.
