Program Director, Targeted Case Management
General
The Program Director for Targeted Case Management for Medicaid eligible clients in accordance with Social
Security Act, Section 1915(g); Florida Medicaid program authorized to work under Florida Statute, chapter 409;
Florida Administrative Code, chapter 59G; and the Mental Health Targeted Case Management Coverage and
Limitations Handbook..
Qualifications
Master’s degree in Counseling, Psychology, Social Work or related Human Services field. Minimum of two years
supervisory experience in social welfare or health care agency. Professional Florida License in Mental Health,
Social Work, Marriage and Family Therapy or Psychology (intern registration will be acceptable). 1 year
experience working with children and families. Must be able to work flexible hours and travel.
Specific Functional Duties
















Supervises case management staff including staff meetings and case consultations; ensure proper client
to staff ratio
Assign cases to case management staff and oversee service delivery
Implement, coordinate and supervise direct service providers
Ensures the targeted case management programs meet all Florida Therapy Services licensing,
contractual and quality assurance standards and requirements
Provide support with in-crisis or problem cases; provide on-going assistance to targeted case
management staff in coordinating services for clients
Provide training to new targeted case management staff and on-going training for existing staff
Review and monitor case files for both internal and eternal quality assurance standards
Establish and maintain positive collaborative relationships with relevant community agencies
Develop and regularly monitor annual program budget
Coordinate and participate in the quality improvement process
Serve as a member of the Florida Therapy Services Management Team
Liaison to community agencies such as, ADM, DCF, and DJJ and attend closure or conference staffing
Ensure targeted case management staff completes all reports in a timely manner
Prepare and conduct staff performance evaluations
Participate in grant writing, contract negotiating or re-accreditation preparation
Conduct monthly clinical chart reviews, peer reviews and customer satisfaction surveys as required

Requirements










Attend and participate in company sponsored trainings and quarterly meetings
Complete ADM approved mental health targeted case management training
Must be able to work flexible hours
Have an active e-mail address and telephone with active voicemail
Complete and/or submit documentation of completion of two hours of HIV/AIDS awareness training in the
first six months of contract date and every two years thereafter
Complete and/or submit documentation of completion of two hours Aggression Control training in the first
six months of contract date and annually thereafter
Complete and/or submit documentation of completion of certified CPR training in the first six months of
contract date and keep current thereafter
Complete CFARS and FARS training on line.
Maintain confidentiality in accordance with HIPAA regulations.
Maintain Continuing Education Units as required by State Licensing Boards.
