DIRECTIONS:
Faculty and staff who receive College support for professional development, conference travel,
research/creative activity, etc., are required to submit a final report to the CVPA Associate
Dean of Academic Affairs. Please complete the form at the top of this page, attach this page to
a one to three page report prepared as directed below, and submit to the Funding Review
Committee c/o Associate Dean of Academic Affairs by the appropriate due date. Reports may
also be submitted electronically to cvpafs@gmu.edu.
DUE DATE:
The report is due one month after the completion of travel, activity, or project unless otherwise
noted on the approved funding request.
