You must be working in a DBT Programme that delivers all functions and modes of DBT for a minimum of 1 day per week. Your team leader will have to sign your application to confirm this. You must have been working in DBT for at least one year. Your team leader's signature on your application will verify this. You must attend a DBT consultation team meeting once a week. This will be verified by your team leader's signature on your application form. You must have completed 450 hours of structured teaching/self study and participation, which will include 70 hours of intensive training in DBT. Direct clinical practice may account for up to 225 hours of the 450 hours. You will be asked to submit a training log. You must have read the Linehan text book and skills training manual. This will be self-certified at present, although in the future there may be a written exam. You must have attended a mindfulness training (retreat, course, community mindfulness group with teaching). You will be asked to submit a log of this. You must have treated at least 4 clients with DBT before you apply for accreditation. Two of the four clients must have been treated from pre-treatment to treatment completion. All four of the clients must have been discussed in consultation team. You will be asked to provide a case formulation for one of these clients. You supervisor will be asked to verify the number of clients you have treated. (If you have done the PG Cert in DBT and received a passing mark on your case formulation you will not need to do this again). You must have had 20 hours of supervised practice on two clients. Some of the supervised practice must include the review of taped sessions by an "accreditable" supervisor (ie the supervisor will had their own tapes rated to adherence, worked in a full DBT programme for more than two years, received supervision themselves and meet all the other criteria for accreditation). You must have experience of teaching all four modules of DBT skills in group and this will be verified by your team leader. You must be able to recognise your own therapy interfering behaviours. You will be asked to provide a chain analysis of one of them as part of the application process. (If you have done the PG Cert in DBT and received a passing mark on your chain you will not need to do this again). You must submit three consecutive tapes to be rated for adherence to DBT. You must be a member of the Society for DBT (if you attend the annual conference your membership costs are included in the cost of the conference).
