ATTENTION ENTERING 7TH – 12TH GRADERS Lakeside Union School District You Could Win Fun Prizes AB354 is mandating that every student entering the 7th - 12th grade must have the Tdap (tetanus, diphtheria, and acellular pertussis) booster for the 2011-2012 school year. Do you realize that pertussis (whooping cough) has taken the lives of 300,000 each year worldwide? Children below the age of 2-3 months cannot have the vaccine and are susceptible to contracting this disease from older children and adults within our community. HOW CAN I WIN? Open to Lakeside Middle School, Tierra Del Sol Middle and East County Academy of Learning students who provide documentation that they’ve met the state mandated requirement for Tdap (booster on or after their 7th birthday). If your child is exempt from vaccinations due to medical reasons or personal beliefs, please contact the health clerk at your child’s school to complete documentation once that is completed your child will be eligible for the contest. Submissions must be received by June 9th to be entered into the drawing. WHAT IF I HAVE ALREADY HAD THE Tdap? Fax, mail or walk a copy of your immunization card showing proof that you are in compliance with the law (e.g. have had Tdap on your booster after the age of 7 years old) to your school’s health office. You will not be able to register or start school unless you provide documentation that you have had the Tdap booster and/or are in compliance with the law.
