Nursing Assistant Scholarship
Scholarship Application
The purpose of the nursing assistant scholarship is to provide students in this program with financial support
who otherwise could not afford to attend this program. It is designed to give those who are financially or
socially disadvantaged the help needed to start or restart their education and reach higher levels of
employability.
Applicant must be accepted into the Nursing Assistant program at MassBay Community College. Completed
applications, including required supporting materials, must be submitted to the MassBay Community College Foundation
office, Room 221, 50 Oakland Street, Wellesley Hills, MA 02481.
Scholarship application must be typed.
Criteria:

Preference given to economically and/or socially disadvantaged US resident /citizen
Supporting Materials Required:




Letter of recommendation from a faculty member, or teacher, or employer, or respected member of the
community. Letters of recommendations will not be accepted from family members.
An Essay
A copy of your high school transcript or Certificate of GED completion or most recent college transcript.
A copy of the most recent W-2 (proof of income) or a copy of the state forms/letters verifying eligibility for subsidiesSection 8, WIC, EBT, SSI.

awarded a scholarship, information about me may be released for publicity purposes. I must meet requirements for
receiving awarded funds. Furthermore, if I am selected to receive a scholarship, I will abide by the conditions of
acceptance set by the scholarship criteria.
