Family Scholars Program Director Form 2010 Thank you for your interest in AMCHP’s Family Scholars Program (FSP). Through support from the Maternal and Child Health Bureau and Centers for Disease Control and Prevention, AMCHP will award scholarships to family representatives to attend the AMCHP Annual Conference. Each applicant is required to submit an FSP Director Form to be completed by a Title V director in their state. Title V includes Maternal and Child Health (MCH) programs and programs serving Children and Youth with Special Health Care Needs (CYSHCN). Your responses to the questions in the form will be considered by the review committee during the selection of family scholars. A Title V director may nominate no more than two individuals as family scholars for consideration; however, only one individual will be selected from a state or territory annually. Note: After the form has been completed, return the form to the applicant. The applicant will submit this form as part of the application to AMCHP. If any forms are missing from an applicant’s package, they will not be reviewed and considered for a scholarship. Only applications submitted by e-mail will be reviewed.
