During my training, I have gained experience in all areas of obstetrics and gynaecology.
Since I joined as consultant in July 2002, I have consolidated my clinical and surgical experience and established my capability of performing all the clinical & surgical duties required of a standalone consultant. I established the urogynaecology service in the trust.
Communication between secondary and primary care teams was poor when I first took over continence care. There was no continence service. We developed an integrated continence care pathway for patient journey from initial GP appointment through to community or secondary care outpatient clinics. The team met fortnightly to discuss practice and swap referrals, and holds regular forums with community continence nurses and GPs, and with urogynaecologists, colorectal surgeons and urologists to develop knowledge about continence care assessment and management.
In 2003, I managed to acquire funding for 1 continence adviser which was subsequently expanded to 4 continence advisers by 2006. Urodynamics service was a shared facility with urology till 2006. Due to the increased demand, I developed an urodynamics service through a business plan with the Strategic Investment and Resource Group
(SIRG) in 2007. We triaged the referrals for urodynamics. This had evolved into fortnightly videourodynamics service in 2009 due to service requirement. Continence
Care Team led by myself won Runnerup ‘Continence Care Team of the Year 2007’ in the Hospital Doctor Awards held on 22nd November 2007 in London.
I introduced new technologies in incontinence surgery (TVT-2002, TOT-2004, Singleincision Mid-urethral tape 2008) and Pelvic Floor reconstructive Surgical Mesh Kits
(Apogee, Perigee -2007; Elevate anterior-2009 and Posterior-2007). All were introduced following approval of the clinical Governance Board.
I was a lead for Pelvic Floor Service across the trust. I chaired the monthly
Multidisciplinary meeting for Gynaecology/Urology. We discussed the complex cases and devised a management plan. I set up a Pan-Birmingham Continence network in
2007. The gynaecologists, urologists, colorectal surgeons and the continence team met once in 3 months to discuss issues and new developments to streamline the service.
I was a risk management lead for the department between 2007 and 2008, we were able to investigate the critical incidents in obstetrics & gynaecology, performed root cause analysis and formulated the recommendations to improve the service. We also provided the feedback to the staff involved and offered debriefing to the affected families to reduce litigation.
As a representative of the Drugs & Therapeutic Committee (DTC) for gynaecology, I introduced the joint Botulinum toxin injection therapy guidelines with the urologists for the intractable detrusor overactivity in 2007 for the trust.
As a college tutor (2004 - 2006) I obtained excellent score at the RCOG Hospital visit in
April 2004.

I had held Clinical Excellence award Level IV since April 2010 till 31 st January 2013.
I am hard working, have excellent leadership skills and possess the quality to motivate others as well.
During my obstetric and gynaecology career, I have worked in different types of hospitals, which include - university, teaching, and district general hospitals, both in
India and various deaneries within the United Kingdom. The different hospitals and wide variation in practice have provided me with comprehensive training, excellent clinical and surgical skills along with sound experience in general obstetrics and gynaecology.
Urogynaecology:
As a urogynaecologist my workload included assessment and management of primary and complicated cases of urinary incontinence and pelvic organ prolapse. I am proficient in assessment, diagnosis and management of stress urinary incontinence, overactive bladder, painful bladder syndrome, voiding dysfunction and uterovaginal prolapse. I am able to perform and interpret all types of Video/Urodynamic investigations: cystometry, uroflowmetry, urethral pressure profile, pressure flow studies and ambulatory studies.
Minimal Invasive Surgery
I have been accredited with Level II Laparoscopic surgery by the RCOG in 2001. I had been accredited as a trainer for Level II Laparoscopic surgery since 2004 and trained 3 trainees for completion of SSM/ATSM since then. I had significant case-load in operative laparoscopic procedures such as treatment of endometriosis (excision and diathermy), adhesiolysis, laparoscopic salpingectomy and oophorectomy. I am proficient in using Ligasure
which enables me to perform Laparoscopic Assisted Vaginal
Hysterectomy in a quicker way.
Obstetrics:
I have completed a broad training programme in obstetrics including pre-pregnancy counselling, antenatal care, intra- and postpartum care which provided me with the knowledge and experience to play an active role in providing safe and effective care for pregnant women. My initial training in busy units in India, albeit being tough, provided me with the necessary skills in management of obstetric emergencies even in the presence of inadequate resources.

I enjoy teaching at all levels, but particularly postgraduates, either individually or in groups. I give tutorials and lectures to each group of medical students as well as teaching them on ward rounds, in clinics and whilst operating.
I was proactively involved in the Problem-Based Learning (PBL) programme for the
University of Manchester in 1999-2000 on teaching of Medical Students. This was a pilot project successfully implemented later on.
As a honorary senior clinical lecturer for the University of Birmingham, I am actively involved in teaching of medical students, and trainees within the West Midlands. I have been receiving excellent feedback from the students. On the wards, I listen to the medical students case histories but direct my teaching towards the junior doctors as well. I encourage active debate and hope that this stimulates interest. Currently I am involved in setting up a revised curriculum for the medical students for the University of
Birmingham. I was a medical student programme facilitator for the University between
2005-2007 (one term).
I developed a elearning module with Prof. Khan on “Training the teachers” for the
University of Birmingham in 2009. This pilot project was successfully implemented to all of the consultant teachers in the West Midlands Deanery.
Local
I enjoyed small group teaching as it allows better communication between the teacher and the students; it is more interactive than formal lectures, encourages discussion and provides greater insight into students’ needs. I was a RCOG preceptor for Urodynamics
- Special Skills Module (SSM), Urogynaecology, Laparoscopic surgery and Benign
Vaginal Surgery - Advanced Training Skill Modules (ATSM)s. I had supervised five trainees in completing these modules since 2005. I had been training GP registrars for primary care management of continence problems.
National

RCOG College tutor (2004-06): I worked as a college tutor for this period. During this time, I obtained excellent score at the RCOG Hospital visit in April 2004.
I was appointed as a Postgraduate Medical Education and Training Board (PMETB) partner / GMC associate and a specialty advisor for Obstetrics & Gynaecology since
October 2008. I held this position till February 2013. My role was to assess the article
14 applications for certification process, ensure quality control and to check accreditation and evaluate the training programmes throughout the UK.
International
As a member of International Urogynecological Association (IUGA) education committee
(2007-2013), I evaluated and reviewed the international fellowship and observership program. I held this position as a chair for workshop subcommittee for 6 years. We evaluated the observership, fellowship and workshop subcommittee applications every year. I was invited to make proposals and amendments for training in Urogynaecology for IUGA.
I have thoroughly enjoyed my role in organising and facilitating surgical workshops where I have demonstrated and taught senior and trainee clinicians on various surgical procedures. I taught these procedures to consultants with an interest in urogynaecology and senior trainees on simulators, cadavers and also in live surgical sessions .
I organised a number of multi-disciplinary tutorials/workshops in urogynaecology for teaching various skills:

I was a lead for the integrated continence team which comprises both the PCTs
(Sandwell and Heart of Birmingham-HOB) and this acute trust. The continence team has streamlined the integrated care pathway for continence care across the Trust.
Communication between secondary and primary care teams was poor when I first took over continence care. By setting up the integrated care pathway, with the community continence team triaging GP referrals, we reduced waiting times for urodynamics clinic from 13 months to six weeks.
The team structure encouraged cross-referral between specialties without the patient

having to go back to the GP. This gave a seamless service and makes the patient’s journey easier.
I chaired monthly MDT-Pelvic Floor Meetings and led the Pan-Birmingham continence network across the trust and the PCTs. We were able to hold case conferences to discuss complex cases that benefit from the combined expertise of the team members.
I am committed to life-long learning and currently participate in the RCOG-CPD program. As a fellow of the RCOG; I regularly receive and read the “Obstetrician and
Gynaecologist” journal, which provide an excellent source for updated reviews for general obstetrics and gynaecology. I regularly receive and review the RCOG guidelines as well as the NICE guidelines. I use this website also to do my literature searches using the Medline and Cochrane databases. I am a member of a number of subspecialty societies and receive their journals, which keep me updated on progress in my areas of interest. I have completed second 5-year CPD cycle in January 2012. I am on 3 rd CPD cycle since March 2012.

In my previous post I have established an ongoing programme of evaluative research in urogynaecology. This had allowed me to acquire experience in applying for funding, seeking ethics approval and conducting research within a framework of research governance.
I had supervised many research projects and three international multicentric projects

Midurethral tape in stress incontinence). I had also secured in obtaining 2 research grants and a major grant as a co-applicant through NIHR on" Ultrasound assessment of bladder wall thickness in women with overactive bladder".

Minnesotta, published in 2011. Secured the research grant and obtained the ethical approval from HREC UK.
Multicenter prospective trial to determine the inter-observer correlation of the simplified POP-Q in describing pelvic organ prolapse for IUGA, published in
2012. Involved with CRF data entry. Coordinated and managed the whole project for the UK.
Bladder Ultrasound study (BUS) for Overactive Bladder for NIHR commenced in
2011 (Ongoing)- Co-investigator.
Devised the research protocol for Local anaesthetic study on Ophira™ for
Promedon Inc., for Canada due to be commenced in 2014. Submitted for ethical approval in Canada.
I have been invited to be a panel member on Diagnostic Technologies and Screening
Panel (DTSP) for National Institute for Health and Research (NIHR) since April 2009.
I have authored and co-authored over 30 publications in peer-reviewed journals.
I have assessed all of these applications for financial viability and monitored the projects once a month.

I was also involved in setting up new guidelines on reviewers’ instructions for this journal. This journal has been made as an official journal for
European Board of Obstetrics & Gynaecology since January 2010. This has helped in improving the impact Factor for this journal.
