Experiential Learning Low Incidence Team TEAM REFERRAL PROCESS Referrals to the Experiential Learning Low-Incidence (ELLI) team are completed by the lowincidence classroom teacher or school administrator. The referral process and placement of students includes the following steps: Step 1 An initial meeting is scheduled in September between low-incidence teachers and the ELLI team. All students will be assessed in terms of skills, work habits, levels of independence and suitability for placement in different experiential learning environments. These learning environments include the classroom (level 1), the school (level 2), and/or the workplace or community-based placements (level 3). The ELLI Continuum will be used in the assessment of each student’s readiness for participation in these three levels of experiential learning. Each student who will be entering an experiential learning environment in the school building (level 2) or in a workplace (level 3) will require the following forms* to be completed and sent to the ELLI office by board mail: 1. The Experiential Learning Low Incidence Referral Form 2. The ELLI Consent Form 1. (Please note that Consent Form 2 will need to be completed when the placement has been established and confirmed.) 3. The Occupational Therapy and Physiotherapy Service Referral Form should be completed if the student poses safety concerns, or if the student has physical/functional difficulties that may affect their placement activities. * Please ensure the correct signatures have been obtained before submitting these forms. Tips: While waiting for the ELLI team to receive and process the forms, teachers are advised to take their students out on a work-site visit or school trip in order to explore the school and local community. During these excursions, students can meet potential employers. Once teachers have made contact with a potential employer for a student’s work placement, they should contact the ELLI team and provide the employer’s contact information. When the ELLI team receives the referral package, they will begin contacting and visiting potential employers in order to set up the placements. Step 2 Once the school, workplace or community-based placements have been confirmed, the following forms need to be completed and submitted: 1. The ELLI Consent Form 2 (must be signed by the student’s parent/guardian before submission). This form can be sent to the ELLI team in the board mail. 2. The Work Education Agreement Form (WEA), must be completed and signed by all parties, including the parent and the employer. The WEA may be kept in the classroom for an ELLI Team member to pick up and complete. Step 3 A member of the ELLI team will contact each teacher in order to set up appointments to complete the Risk Management Assessment and develop a Student Learning Plan that will meet each student’s individual learning needs in the classroom, school and workplace.
