Practice.

and the clinical lead for the Practice.
Dr James Ojidu, MB BS FWACS MRCOG DFFP, MRCGP, PGCert CVM, 1982
Ibadan, Nigeria. Special interest in drug misuse, the Practice representative at
Clinical Commissioning Meetings and also does sessions out of hours.
Dr Robert Chisabingo, DFSRH MRCOG MBCHB Bsc (HB), 1981 Zambia. Special
interest in minor surgery, one of the prescribing leads for the Practice and
deputy Practice representative for the Practice at Clinical Commissioning
Meetings.
Dr Deepak Gone (Male) MBBS, MRCGP. Special interest in musculoskeletal
conditions.
Dr James Mbugua (Male) MBCHB, MRCGP, Diploma in Practical Dermatology
(Cardiff University), Certificate in Diabetes (Warwich University) with a special

The practice have been incorporated by Agencia, and Agencia’s role is in
the daily running of the Active Recovery service. Which is as below
Agencia’s role is to host, coordinate and support service integration and
drive forward the psychosocial element of rehabilitation of the Active
Recovery service. Agencia’s role will also include checking that each GP
practice has appropriate registration with CQC for the Active Recovery
service as part of our overall governance and auditing.

Brief Description of Locations:
Trent View Medical Practice consists of four surgeries located in Crowle
(dispensing Practice), Skippingdale, Keadby (dispensing Practice) and we care
for over 12,000 patients. The Practice provides a service to residents of urban
areas of Scunthorpe at two sites and the Isle of Axholme for the other two
sites and covers a sixty mile radius.
The Practice area covers the following villages in the Isle of Axholme: Keadby,
Burringham, Gunness, Althorpe, Luddington, Garthorpe, Fockerby, Adlingfleet,
Crowle, Eastoft and Ealand. We also cover areas in Scunthorpe, around Ferry
Road West (predominantly comprising of the Skippingdale and Berkeley
Housing Estates). This is bordered by the A1077, Doncaster Road and a line
running from Kingsway Gardens behind Berkeley and St. Augustine Webster
Schools and through Atkinsons Warren to the A1077.
The Practice and its Philosophy:
This GMS (General Medical Services) Practice has existed for over 60 years
operating from Practice owned premises. The Practice has a current list size
of approximately 12,000, which is remaining stable and is part of the North
Lincolnshire CCG (Clinical Commissioning Group). QoF (Quality and Outcomes
Framework) points have remained steady for two years. There is deprivation
in some areas of Scunthorpe, but the patient population is very varied and
covers both rural and suburban areas. The Practice continues to aspire to
meet the challenges and targets of the NHS and NSF (National Service
Framework) requirements.
The Practice is committed to improving working lives and is an equal

The Primary Health Care Team works as a cohesive unit and has particular
strengths in the relationship with District Nurses with whom everyone works
very closely.
The Partners are committed to a patient centred, non-discriminatory approach

on high quality care. The Practice presents as a very friendly, open,
democratic and “down to earth” team with a flexible approach and very low
staff turnover.

The Practice also takes part in the Shared Care Scheme (drug addiction
therapy) and offers support for the CIC (Community Intermediate Care) bed
scheme. We also provide services which are non NHS and are paid for by the

For patients that do not speak English a family member can translate at their
request or a translation service is available. We are also able to provide
information in larger fonts or electronically as requested.

Confidentiality and Access to Patient Information
All patient information is considered to be confidential and we comply fully
with the Data Protection Act to keep your confidential data safe and secure.
All employees have access to this information in relation to their role and
have signed a confidentiality agreement. Relevant information may be shared
within the multi-disciplinary health care team at the Practice and with other
health care professionals to whom a patient is referred. Those individuals
have a professional and contractual duty of confidentiality. Confidential and
identifiable information relating to patients will not be disclosed to other
individuals without their explicit consent, unless ordered to do so by court
order or where it is in the public interest to do so to prevent harm.
The Data Protection Act and The Access to Health Records Act allow a patient
to find out what information is held in their health records and those of
someone who has died in some circumstances. A patient wishing to see
health records should make a written request to the Practice. Where entitled
to receive a copy, it should be noted that a charge will be made.
Comments, suggestions and complaints
We welcome comments and suggestions on our service and have a protocol
for dealing with complaints. Complaints should be made to the Practice
Manager, who will ensure all relevant details are recorded, arrange for the
complaint to be investigated and respond in a timely manner. If the complaint
is not resolved to the satisfaction of the complainant the Health Service
Ombudsman can investigate the complaint further.
