disabled patients only, we ask all our other
patients to walk when possible, or use a cark
park in the immediate area
As a patient you have a right to:
 Be registered with a G.P.
 Have a full health check when you join a doctor’s list.
 Have emergency care when you need it.
 Have appropriate drugs prescribed.
 Be referred to a Consultant acceptable to you when
your G.P. feels it necessary.
 Have access to your own health records (subject to
the law) and to expect them to be maintained
confidentially.
 Not be discriminated against on the grounds of race,
gender, social class, religion, sexual orientation or
appearance, disability or medical condition.
Please Note – this Practice contributes anonymous data
used for health care research purposes. The data collection
scheme has been approved by the South East Multicentre
Research Ethics Committee, and the Committee will be
overseeing all research conducted on that data.
The database will hold data on millions of patients, and no
patient will be traceable to anyone outside this Practice.

Welcome to our Practice
We hope the following information will be helpful. The
Hermitage is an old established Swindon G.P. Surgery now
working from a purpose built building in Old Town. Our aim
is to provide the highest standard of health care, and this can
be best achieved by developing close partnerships between
doctors, surgery staff and patients. We always welcome
comments or ideas for improvements.
Opening hours:
08.30 to 18.00 Monday to Friday, (closed Wednesday 13.00
to 16.00 for staff training) with extended surgeries on some
days.
We have disabled access and toilet facilities.
Surgery Hours:
Surgeries and clinics are spread so that the Doctors and
Nurses are available throughout most of the opening hours.
All surgeries and clinics to see both Doctors and Nurses are
by appointment. Reception is open from 08.30 to 18.00. If
you can’t keep your appointments, or need to change it
please telephone Reception as soon as possible so that
someone else can be offered your place.

Out of hours Emergencies:
If you have an urgent problem when the surgery is closed,
you will be directed via our phone number to NHS 111.
Repeat Prescriptions:
We need these in writing, ideally with the tear off slip from a
previous prescription, ticking which items you require. You
can also FAX your requirements to us (01793 512520). If
you enclose a stamped self-addressed envelope we will post
your prescription back to you, or most Pharmacies will now
collect from us and deliver to your door.

Please allow up to 48 hours for repeat prescriptions
to be processed.
Home Visits:
If you need a Doctor to visit you at home because you are
too ill to travel to the Surgery, please make your request
before 10.00 a.m. if possible (except in an emergency) with
full details.
