We have twelve doctors covering the two surgeries. We are a GP training practice providing a wide variety of services in a modern and friendly environment. Bookable appointments are normally available everyday between 8:30 – 10:00 and 2:30 - 5:20. The OPEN SURGERY will be between the hours of 11:00 a.m. and 12:00 noon Monday to Friday. You do not need to phone and book in advance for this doctor clinic, just present yourself at reception on arrival between these hours. The appointments will be booked in time order on patients’ arrival at surgery. Please be aware due to patient demand there maybe a wait but we do guarantee that you will be seen. You cannot request a specific GP. You will be seen by any of the GP’s running the clinic on the day you attend. If you do wish to see the doctor of your choice, please book a routine pre-booked appointment. They have been trained to undertake a wide variety of tasks that ensure the smooth running of our services and assist you to make the best use of the services available. PRIMARY HEALTH CARE TEAM The practice has excellent links with members of the Primary Care Team in the area. These include district nurses, phlebotomists, health visitors, midwives, Macmillan nurse, school nurses, social workers and community nurses, all of whose services can be obtained upon request. Additionally, we can arrange in-house physiotherapy, dietetics and counselling services, when recommended by your doctor. CONFIDENTIALITY Whilst you remain registered with this practice, details from your medical records may be released to other health professionals unless you instruct us otherwise in writing. This information will only be released in order to enable you to receive proper care and treatment or to help protect the health of the general public. PARKING AND DISABLED ACCESS Parking is available at both premises. There are blue-badge holder bays adjacent to the building and ramped access to the front door from there. All the consulting rooms and toilets are on the ground floor and accessible by wheelchair. TRAINING PRACTICE The Marches is a training practice for registrar doctors who wish to become GPs. They are fully qualified and are broadening their experience, prior to becoming GPs. Occasionally, we may ask your permission to video record your consultation; it will only be reviewed by the registrar and the GP trainer who is a member of this practice. PATIENTS BETWEEN THE AGES OF 16 AND 74 If you have not been seen within the last three years you can request and receive a three-year review. PATIENTS 75 YEARS AND OVER If you have not been seen within the last year you can request and receive an annual review. PRACTICE BOUNDARY We have a practice area covering Buckley and Broughton NON SMOKING We operate a non-smoking policy on the premises. MOBILE PHONES Mobile phones may NOT be used on the premises. REGISTERING AS A NEW PATIENT Please ask a receptionist for a patient registration form. The registration procedure will be explained to you. COMPLAINTS If you have a complaint about any aspect of the service we provide, please first ask to speak to a supervisor. Then, if necessary, the complaints officer will be contacted. They will investigate the matter and aim to resolve the problem with you as soon as possible. A leaflet which explains the procedure is available; please ask for one from reception. INTERNET ACCESS We now offer the facility to book appointments, order repeat prescriptions, update address and telephone details via the internet. Please ask a receptionist for more information. CONSULTING YOUR DOCTOR Doctors see patients by appointment. A proportion of their appointments are prebookable up to four weeks ahead. For an appointment on the day we have an open surgery from 11:00 to 12:00. TO MAKE AN APPOINTMENT Appointments can be made by telephone, visiting reception during opening hours or via the internet. Doctors’ and nurses’ time is precious. Please always keep your appointment or contact us in good time if you will be unable to attend. Patients are welcome to choose which surgery they wish to attend, but we ask that you only contact and use the surgery you have chosen for all appointments and general enquiries because this will be where your records are held. When booking an appointment you are free to request one with the doctor of your choice. You may expect to be offered one so long as there are vacancies. NURSE-LED EXPRESS CLINICS We run specialist nurse-led 'express' sessions on most days to deal with minor illness. We have nurses who have received additional specialist training in this area of clinical care. * They will be able to help you with many common problems. You can either book a face-to-face appointment to see the specialist nurse in surgery or book an appointment time to speak to her on the telephone. * A list of the common problems that the nurse can deal with is shown below. COMMON PROBLEMS LIST - EXPRESS CLINIC Coughs and colds Flu and simple viral illnesses Earache Morning-after contraception Red sticky eyes Rashes Sickness and diarrhoea Cystitis and thrush Back pain Baby problems such as nappy rash Sore throat Constipation Vaginal discharge Sprains and minor injuries OTHER NURSE-LED CLINICS Many of your routine health and review checks will be performed here by the nurses, thus giving the doctors more time to make the Advanced Access appointment system work and see patients with more complicated or serious conditions. Our practice nurses have appointments for the following: Minor illnesses Asthma clinics Heart disease checks Stop smoking advice Minor surgery Cervical cytology Contraceptive advice HRT advice Child immunisations Blood pressure checks Well man clinic Well woman Travel vaccinations and advice Diet advice Diabetes clinics OUR HEALTH CARE ASSISTANT Has undertaken specific courses and training to perform a variety of nursing tasks for example you may see her for: Blood pressure checks, have blood samples taken, have an ECG, routine monitoring, i.e. patients on Warfarin and patients who are taking immunosuppressant drugs HOME VISITS Please phone the surgery before 10.00am if possible. Home visits are reserved for patients who are too ill to attend surgery. They are extremely time consuming and we generally follow the guidelines laid down by NHS Wales, which has approved the following: * A GP visit is recommended to terminally ill and bed-bound patients. * A GP visit may be useful in cases of chest pain, severe shortage of breath and severe bleeding. If a GP is informed that a patient has symptoms that suggest a heart attack, the sensible approach may be to dial 999 and call an emergency paramedic ambulance rather than attending the surgery. ZERO TOLERANCE We strongly support the NHS policy on zero tolerance. Anyone attending the surgery who abuses GPs, staff or other patients (be it verbally, physically or in any threatening manner whatsoever) will risk removal from the practice list. In extreme cases we may summon the police to remove offenders from the practice premises. OUT-OF-HOURS SERVICE Betsi Cadwaladr University Health Board is responsible for commissioning the out-ofhours service which is run by the health board and operates between the hours of 6.30pm and 8.00am Monday to Thursday and from 6.30pm Friday until 8.00am Monday (Tuesday during bank holidays). There is always a doctor on call and you may be asked to attend the out-of-hours emergency centre. The phone number for this service is 0300 1235566 Patients might also consider phoning NHS Direct, a 24-hour helpline, on 0845 4647 at any time for medical advice, or visit them online at www.nhsdirect.nhs.uk OTHER SERVICES Appointments are available to see the district nursing sisters for dressings etc. These are usually done at the local clinic. A phlebotomy service (taking blood samples) is available daily; an appointment is necessary. Antenatal clinics are held weekly in association with the community midwives. Details of these clinics are available from reception. REPEAT PRESCRIPTION ORDERING Requests for repeat prescriptions are made by using the re-order slip that is issued to you with every repeat prescription. You can do this either by delivering it to the surgery, posting it, faxing it to us or via the internet. For safety reasons we are not able to accept any medication requests via the phone. We require just two working days to prepare your repeat prescription. Some local pharmacies offer a prescription collection service and, with your authority, they may be able to collect your prescription from us on your behalf. Please speak to your local pharmacy to arrange this. Please be aware that using the service provided by a local pharmacy means all of your prescriptions goes the pharmacy you first signed up with, should you change to a different pharmacy please let us know so we can update our records. TEST RESULTS Please only telephone the surgery after 2.00pm to enquire about test results. Your doctor will let you know how long it usually takes results to be returned to us. Please note that X-ray results take up to two weeks; cervical smear results usually take between one and two months. SICKNESS CERTIFICATES For any illness lasting seven days or less, patients should obtain form SC2 (Self Certificate) from his or her employer. Medical sickness certificates for an illness lasting longer than seven days will be provided by your GP. Certificates issued for less than seven days involve non-NHS work and will attract a charge. CERVICAL SMEARS We recommend that all women up to the age of 65 should have a smear test every three years. You will be recalled for this by the Health Authority. Please make an appointment with a practice nurse. FLU VACCINATIONS We recommend influenza vaccination for all patients over the age of 65 years and for patients considered to be at special risk. Flu vaccinations are usually given in the autumn; details for the clinics will be displayed in reception. TRAVEL VACCINATIONS Travel clinics are held weekly. Please contact us at least eight weeks before travelling in order that the vaccination plan may be worked out for you. There are charges for some vaccinations. Details of these are available from reception. YELLOW FEVER CENTRE Our Broughton Surgery is a registered Yellow Fever Centre. Please phone us for details. FAMILY PLANNING A doctor's appointment is not necessary to discuss your family planning requirements. Please make an appointment with one of our practice nurses. A family planning clinic takes place every week at Broughton and at Buckley. Please phone for an appointment. MEDICAL EXAMINATIONS AND PRIVATE FORM COMPLETION A British Medical Association recommended fee will be charged for undertaking medical examinations and for the completion of non-NHS forms. These include: private certificates, holiday cancellation forms, insurance forms including private healthcare, employment forms, HGV, taxi and elderly driver medicals.
