DCT POSTS IN COMMUNITY DENTISTRY AND ORAL SURGERY /ORAL & MAXILLOFACIAL SURGERY Sheffield Teaching Hospitals NHS Foundation Trust Charles Clifford Dental Hospital Barnsley District General Hospital APPOINTMENT Two full time posts at the level of DCT (Dental Core Training) are available in Community Dentistry and Oral Surgery/Oral and Maxillofacial Surgery. These DCT posts are approved by the Postgraduate Dental Dean. Applicants should have completed Dental Foundation Training, or equivalent, having completed periods of training in both primary and secondary care. It would be advantageous to have passed one or more parts of the MFDS or MJDF exam, but these are not essential requirements. This post therefore builds on dental foundation training. The post provides suitable experience for those trainees wishing to complete the MFDS/MJDF examinations of the Dental Faculties of the UK Surgical Royal Colleges. Opportunities exist within the rotation to gain exposure to Orthodontics and Restorative Dentistry during the Barnsley phase of the rotation. Consideration would be given to special arrangements for a post-holder wishing to improve knowledge and experience in other dental specialties subject to approval and compatibility with service requirements. COMMUNITY Function of the DCT Community Dental Practice Post This post provides training and experience in clinical community dentistry, and is intended to support candidates preparing for the MJDF examination. The post is approved by the Hospital Recognition Committee of the Faculty of Dental Surgery, and will count as a period of General Professional Training. The duties comprises three main elements: Clinical community dental practice. The postholder’s own clinics undertaking work of a complexity expected of clinical dental officers. The work on these sessions will comprise of general dental care for relatively non-complex patients, including domiciliary care. Observation and selected clinical work under direct supervision, of a range of more specialised work on patients with complex social and medical histories undertaken by senior community dentists. Formal and informal periods of study. The trainee will be a member of staff of the Community and Special Care Dental Services, and will be accountable on a daily basis to Senior Community Dentists and Consultants in Special Care Dentistry. The formal training programme will be managed by a nominated senior clinician. Duties and responsibilities Provision of dental treatment to patients with in the remit of the Community and Special Care Dental Service in dental clinics, mobile clinics and domiciliary settings, including the driving between establishments and the transportation of appropriate equipment required for such visits. The completion of associated clinical records and statistical returns. Provision of preventive care and advice to individual patients and their carers. Occasionally this may also include the provision of oral health promotion services to groups of such patients, or their carers or care staff. Provision of dental screening at appropriate establishments, including schools and residential and nursing homes, including liaison with education and care staff. In conjunction with the nominated senior clinician, arrangement and completion of an agreed training programme in clinical community dentistry, to include the following: - - Attendance at senior community dentists’ clinics. - Clinical practice for a range of patients with special dental needs. - Attendance at consultant clinics at the Charles Clifford Dental Hospital. - Participation in Oral Health Promotion training programme. - Participation in Dental Public Health’s training programme. - Completion of a specific audit project related to community dental practice. Undertake formal and informal periods of study, to include:One formal study session per week. This usually includes, participation in tutorials as arranged by members of the Dental team and the Charles Clifford Dental Hospital. 2 - Preparation for one or more parts of the MJDF examination of the Royal College of Surgeons of England (or alternate college equivalent). - Whitley Council terms and conditions of service for SHO study leave allowances will apply for all study leave applications. To hold a series of regular seminars to review progress with the nominated senior clinician managing the training programme. Liaison, as appropriate, with hospital and general dental service colleagues and other Community Health Service staff in order to co-ordinate the patient services provided by the Personal Dental Service with those of other dental and community health services. Direction and supervision of Professions Complimentary to Dentistry including dental therapists, dental hygienists, dental health educators and dental nurses. Participation in dental audit, clinical governance and Trust and service training events organised for all staff, including the Trust’s mandatory training programme. Maintenance of a record of your own continuing professional education and development including the experience gained during tenure of this post. SPECIFIC INFORMATION & PRINCIPAL DUTIES To work within the OMFS team framework in providing quality patient care. To carry out specific duties allocated by the Senior Staff and/or suggested by Higher Surgical Trainees (SpR) or Specialty Dentists, in outpatients, wards, and theatres and provision of emergency care at any of the hospitals in the rotation. This may entail travel between the Royal Hallamshire Hospital, Charles Clifford Dental Hospital, Sheffield Children's Hospital and the Northern General Hospital. The nature of the rotation is such that for the purposes of continuity of care, and fairness to all post-holders, staff will be expected to carry out duties at all the sites listed - assistance with finding accommodation may be available when on the Barnsley phase of the rotation. Exceptionally, the post-holder may be required to travel to another of the rotation hospitals to carry out duties other than those specifically listed for that phase of the rotation. This might occur in circumstances of staff illness or unforeseen difficulties. One protected study session is provided in the weekly timetable. During this session a structured educational programme will be provided which will include clinical update and some topics aimed at the MFDS/MJDS Exam. It should be noted that experience gained prior to obtaining the Final Fellowship exam or parts A, B and C of the new MFDS/MJDS does not, as yet, generally count towards higher training. Every DCT post has a named Educational Supervisor and undergoes both hospital and departmental induction on arrival. Trainees are required to sign an educational agreement and each appointment will be subject to regular review by the NHS Trust and the Postgraduate Clinical Tutor to assess satisfactory work and progress. CONTRACTED HOURS 48 hours per week whilst based in Oral Surgery/Oral and maxillofacial Surgery (6 months duration ) and 37.5 hours/week whilst based in the Community and Special Care Dental Service. 3 ON-CALL COMMITMENT - EMERGENCY DUTY ROTA (OMFS) – ONLY FOR THE 6 MONTH PERIOD WHILST ATTACHED TO ORAL SURGERY/ORAL& MAXILLOFACIAL SURGERY The trainees will work a full 1:10 shift system on-call rota which include duties both during the week and at weekends - based in the Head and Neck Unit at RHH. There may be restrictions on authorisation of Annual Leave’ which leaves things open for turning it into fixed leave at a later date if necessary. A separate system operates within the District for dental emergencies. In addition, to the RHH on-call the trainee’s also attends the Sheffield Children’s Hospital for emergencies as all children with paediatric out of hours emergencies will attend the Sheffield Children’s Hospital. It is a condition of employment that the posts are resident in the hospital when on-call and during the night shift, and an on-call room will be provided on the Head and Neck Unit. TEACHING PROGRAMME There is an active Postgraduate sessional series, currently held every week, with a varying but regular menu of journal clubs, clinico-pathological conferences, radiology seminars, invited internal and external speakers, personal progress review, clinical study programmes and practical tuition in ward and surgical techniques. Regional study meetings are held three or four times annually in Oral & Maxillofacial Surgery and Restorative Dentistry. Currently, clinical audit/effectiveness meetings run monthly and post-holders are encouraged and expected to conceive, develop, formalise, execute and present one substantial audit project during the tenure of the post. We also have a strong tradition of research and the Trust leads on a number of key studies including prostate cancer, diabetes, breast cancer and chronic chest disease. An NHS Foundation Trust We became one of the first NHS Foundation Trusts in the country on 1st July 2004. NHS foundation trusts have greater freedom from central government control and the ability to work with their community and staff to set local health priorities. Our Hospitals Although the Trust has overall management responsibility for the services provided, our hospitals each have their own history, which we are rightly proud of. Between them they offer just about every kind of service available in the modern NHS. The Northern General is the largest of our hospitals with 1100 beds. Located to the north of the city, the hospital is spread out over a large site. The Royal Hallamshire is a major acute teaching hospital based around a tower block of 800 beds and a large outpatient department. The hospital also has a minor injuries unit and an NHS Walk-InCentre for emergency care. Linked to the Royal Hallamshire by a footbridge, the Jessop Wing is Sheffield’s 216 bed women’s hospital. Opened in February 2001 it has brought together all of the city’s obstetric, gynaecology and neonatology services in a state-of-the-art, purpose built, £24 million building. Almost 7000 births take place at the Jessop Wing each year. Weston Park is one of only three purpose built specialist cancer hospitals in the UK and provides a full range of non-surgical cancer services to the population of South Yorkshire, North Nottinghamshire and North Derbyshire. It also enjoys national and international recognition for certain specific treatments. It has 140 beds and is home to a specialist cancer research centre and one of the UK’s only teenage cancer units. The Charles Clifford is a dental teaching hospital linked to The University of Sheffield’s School of Clinical Dentistry. The hospital provides dental outpatient services and emergency dental services for the city. 5 As a teaching trust, all of the hospitals have close links to the University of Sheffield, home to Sheffield’s medical school, and Sheffield Hallam University – which provides nurse training. It is a major teaching centre for future health professionals. The dental teaching hospital is linked to the School of Clinical Dentistry, which has an intake of 75 undergraduates per year and is currently expanding its postgraduate and research activities. The School moved into a new building with greatly improved facilities in 1991 and has currently completed an additional building programme to further enhance the research facilities and add extra office space. 6 The hospital is situated approximately one mile west of the city centre, adjacent to the Royal Hallamshire Hospital Teaching Hospital/Medical School complex, as well as being within the University campus. The hospital provides the dental outpatient services for the Sheffield catchment area, and in addition provides specialist services in all departments to patients from a much wider area. It also incorporates the Schools of Dental Hygiene/Dental Therapy and Dental Nursing. There are four NHS and University Departments in the Charles Clifford Dental Hospital and School of Clinical Dentistry: Oral Health and Development (including Orthodontics, Paediatric Dentistry and Dental Public Health) Oral and Maxillofacial Medicine and Surgery (including Oral Medicine and Dental and Maxillofacial Radiology) Oral and Maxillofacial Pathology (including Oral & Maxillofacial Histopathology and Microbiology) Adult Dental Care (including Prosthodontics, Periodontics, Endodontics and Dental Materials Science) Sheffield Children's Hospital This is a teaching hospital containing 148 beds, situated approximately 300 yards from the Dental Hospital. The hospital has recently completed phase 1 of an extensive rebuilding and redevelopment programme including new operating theatres and some new wards. Two inpatient beds are allocated for Oral and Maxillofacial Surgery but more facilities are available on the day case unit together with beds elsewhere for medically compromised patients. There are currently two regular weekly operating sessions. Each week one operating list is dedicated to Oral & Maxillofacial Surgery and the other is mainly for paediatric dentistry and some oral surgery. Barnsley District General Hospital The Hospital The Barnsley District General Hospital serves a population of 222,000 and has a complement of 664 beds. Situated within a mile of the town centre, it is a modern development opened in 1977. There is a block of theatres, together with an intensive therapy unit and other supporting services. Pathology services are provided in purpose built laboratories. There is a medical physics department, a department of physiological measurement and full rehabilitation facilities. The district has close links with Sheffield University Medical School and the Charles Clifford Dental Hospital in Sheffield. There is a new department consisting of five fully equipped surgeries, one of which is designated as an orthodontic surgery. There is an x-ray room with OPT machine and Celphalosat and a large, well-equipped laboratory. HOSPITAL DENTAL SERVICES - ORAL AND MAXILLOFACIAL MEDICINE AND SURGERY The Department of Oral and Maxillofacial Medicine and Surgery encompasses the disciplines of Oral and Maxillofacial Surgery, Oral Surgery, Dental and Maxillofacial Radiology, Assessment and Casualty for Adults, and Oral Medicine. The majority of the outpatient work, both for clinic visits and procedures under local anaesthesia is carried out at the Charles Clifford Dental Hospital. 7 In-patient admissions are to one of the three other hospitals. The provision of day stay surgery is at the Charles Clifford Dental Hospital, the Day-Stay Unit of the Royal Hallamshire Hospital and in the Day-Surgery and Endoscopy Unit at Barnsley DGH. Technical services are provided from the Northern General Hospital and Barnsley DGH where the maxillofacial laboratory staff provides support for all the hospital units in the integrated OMFS service. HOSPITAL DENTAL SERVICES - RESTORATIVE DENTISTRY The Department of Restorative Dentistry encompasses the disciplines of Prosthodontics, Endodontics and Periodontics, and Dental Materials Science. Excellent technical facilities are available within the Dental Hospital. SALARY The whole-time salary scale is at present £27,798 per annum, rising by annual increments to a maximum of £38,705 per annum in accordance with current nationally agreed salary scales. (Salary scales effective from 1 April 2010). In accordance with HSC 2000/031 junior doctors have a contractual obligation to monitor hours on request. This will include participation in local monitoring exercises at Directorate level and STH Trust junior doctors hours screening exercise undertaken twice yearly. POSTGRADUATE TRAINING The Region is committed to developing postgraduate training programmes for both general and higher professional training as laid down by Colleges and Faculties, and by the Postgraduate Dean’s network. At local level, college/specialty tutors work with unit postgraduate clinical tutors in supervising these programmes. Trainees will be expected to take part in these programmes (including Audit) and to attend counselling sessions/ professional review. Study Leave will form part of these educational programmes and will be arranged in conjunction with the clinical tutor. STUDY LEAVE Study Leave is granted in accordance with the recommended allowance subject to the maintenance of the service and to prior approval of the Postgraduate Clinical Tutor at the appropriate Trust. Study Leave is defined as leave taken to attend a formal course of study designed to further the employee's career and to enable the employee to obtain his/her professional qualifications or gain necessary expertise in order to carry out his/her job. Wherever possible employees should attend locally organised courses (i.e. within Trent Region). REGISTRATION Registration with the General Dental Council is compulsory. 11 OTHER FACILITIES Sheffield is well served by public transport with bus stops nearby each hospital. The Dental Hospital has a staff common room and limited canteen facilities, though dining rooms at adjacent hospitals can be used. The residential base hospitals each have a medical mess, together with mess committees that are available to voice any grievances to the hospital concerned. Within the Dental Hospital there is a junior staff committee whose representative sits on the CCDH Clinical Staff Committee to represent the interests of the junior members of staff.
