Co-operative Education Contract

 Attend my Co-operative Education Work Placement regularly and follow agreed
to procedures for absences this semester to accumulate
hours for
successful completion of the program.
 Report to both my supervisor,
, and my Coop
teacher
, of absences, known or unknown.
I understand that failure to do so will result in withdrawal from the Co-operative
Education course and subsequent loss of credits equal to the uncompleted
hours in the program. Initials:
I understand too, that I must:
 Demonstrate excellent work habits at my placement, including following
instructions, working in a safe manner, and treating employers, supervisors, coworkers and others at the placement with respect at all times. Failure to do so
WILL result in removal from the Coop placement, and possible loss of Coop
credits.
I know, as well, that I am to:
 Submit completed weekly logs in a timely fashion, as their completion affects
not only my work habits, but the time and tasks of my employer/ supervisor and
Coop teacher.
As a Coop student, it is my responsibility to be safe, trustworthy, accountable,
respectful, representing S.D.S.S. with integrity.
