Established perioperative ischemia as best predictor of adverse events after
surgery, 3 fold increase in adverse cardiac outcomes in those pts that had it,
defined as a >0.1 mV ST depression or >0.2 mV ST elevation in a three lead
Holter monitor lasting for at least one minute. Periop ischemia gives a 28 fold
increase in cardiac complications within 6 months after surgery, 15 fold increase
at 1 year, and 14 fold increase at 2 years. 70% of long term adverse outcomes
were preceeded by periop ischemia at least 30 days before the adverse event.

Mangano 1996 NEJM is the landmark first RCT showing use of BB before and after
surgery decreased mortality in non-cardiac surgery by 2 fold. Wallace Anesth 1998
showed BB decreased periop ischemia by 50%. Also safe and cost effective.
