We aim to provide efficient, effective and sympathetic healthcare to approximately 3,200 registered patients. We offer traditional values combined with the best medical and administrative practice. Practice Area The Practice serves Aldborough and surrounding villages within a ten mile radius, please see the Practice boundary map at the rear of this leaflet for more information. Historically, there has been some overlap with the neighbouring Practices of Cromer, Sheringham, Aylsham, Holt, Paston and Birchwood. Please be prepared to wait. Monday to Friday 8.30am to 11.00am - Appointments 2.30pm to 5.30pm - Appointments Surgery Times - Nurses Monday to Friday 8.30am to 12.30pm – Appointments 2.00pm to 5.30pm - Appointments Staff Training The Practice and Dispensary will be closed between 12.00 and 2.00 pm every third Thursday of the month for Staff Training. Services / Clinics provided by Nurses The Nurses provide general nursing care such as dressings, ear syringing, suture removal, child and adult immunisations, travel information, routine reviews of medication, cervical smears, pre-pregnancy and contraception advice. They also provide routine NHS Health Checks, Well Man / Woman Checks and a Health Promotion programme which includes: Asthma, COPD, Diabetes, Hypertension, CKD and Epilepsy clinics and general health care advice. Registration of New Patients If you are a new patient wishing to register, you will need to collect or download from our website a registration form, including a brief questionnaire. If you need to see the Doctor this will be arranged. Please see the back of this leaflet for our Practice boundary for accepting new patients. If you are outside of this boundary we are still able to register you as an ‘Out Of Area’ patient, more information regarding this is to the rear of this leaflet. If you are unsure of your eligibility or have any queries regarding your registration, please telephone or visit the Surgery. Telephone Advice Queries: Doctors are happy to discuss certain matters over the telephone. The receptionist will take your name, telephone number and a brief account of the problem or advice needed and the Doctor will return your call when available. The best time for you to telephone is before 10.00am daily. Results: Nurses are happy to discuss results, queries and certain matters over the telephone. They are available from 11.30am to 12.00am daily. Results will normally be given only to the patient concerned. Please allow at least three days for your results to be processed. Emergencies In an emergency, telephone 999 or for urgent medical advice call the Out of Hours Service on 111 or 01603 481253. Home Visits If you need the Doctor to visit you at home, please contact the Surgery before 10.00am. It will help us to plan visits if you are able to give the receptionist some details about your condition and the urgency of the problem. Children who are ill can usually be brought to the Surgery where they will be seen as quickly as possible. Repeat Prescriptions and Dispensary To order repeat prescriptions please telephone Dispensary between 2.00pm and 4.00pm daily, or 9.00am and 10.30am Monday and Wednesday. As we do not carry all items in stock we do need 48 hours’ notice, excluding weekends and Bank Holidays. Alternatively you can return the computerised slip, indicating which drugs you require by using the post box outside the Dispensary door. You may also order repeat medications on SystmOnline or e-mail: aldborough.dispensary1@nhs.net. We also offer a pharmacy collection service for non-dispensing patients, contact details of these Pharmacies are to the rear of this leaflet. To collect repeat prescriptions come to the Dispensary between 8.30am and 6.00pm daily. Prescription Charges – if you are exempt from charges, please show your current exception certificate on collection of medication. Application forms for ‘certificates of exception from charges’ can be obtained from the Dispensary. For those who pay and have a number of medications, it may be more cost effective to apply for a pre-payment certificate. If you require information on this or any other aspect, please ask at the Dispensary. All repeat prescriptions are subject to a review. Fit Notes / Sick Certificates A Doctor’s certificate is only required for periods of continuous absence from work of seven days or longer. For absences of three to six days, a self certificate should be obtained from your employer. No certificate is required for three days or less. Please note: certificates required from the Doctor for the first seven days of sickness will be subject to a fee. What to do if someone dies During working hours, contact the Surgery who will be able to give you further advice. At all other times, contact your local funeral director who will be able to give both help and advice. Community Based Health Staff District Nurses, Community Matrons, Midwifes and Health Visitors can be contacted via the Surgery. Contact details can be found on the back of this leaflet. Fees for Private Medical Services The NHS does not cover the cost of providing certain forms, letters and vaccinations. Fees which are set in line with BMA recommendations are displayed on the Surgery notice boards and are available upon request. Support to the Practice The Practice wishes to acknowledge the support given by members of the local community and, in particular, the many people who have contributed to the Equipment Fund. Donations to the fund have provided and maintained medical equipment which would not otherwise have been available. Services / Clinics provided by the Practice: Family Planning Contraceptive advice and reviews are offered by the Doctors and Nurses. Antenatal Clinic A midwife-run clinic is held on alternate Wednesdays 8.30am – 1.30pm at the Surgery - by appointment only. Baby Clinic / Immunisations The Health Visitor runs an informal ‘drop in’ clinic from 2.30pm to 3.30pm every third Wednesday of the month. This is for baby weighing and general advice. Arrangements to be seen may also be made directly with the Health Visitor who can be contacted through the Surgery. Baby Immunisation clinics are held at the Surgery every 2 weeks (the first Friday afternoon of the month and the third Wednesday afternoon of the month); once you have received your letter from the Health Surveillance Team you can contact the Surgery to make an appointment. If you have any queries, please ask to speak to one of our Nurses. Children It is recommended that children up to five years of age should be registered for Child Health Surveillance. This is a programme run by the Doctor, Health Visitor and Practice Nurse and includes a baby check and immunisations. Travel Advice and Immunisations Please ask the receptionist for the appropriate form. There is a charge for some travel immunisations. Charges will be made for temporary residents. North Norfolk Clinical Commissioning Group and Iceni Healthcare This Practice is pleased to be working in close co-operation with other local Practices under the auspices of North Norfolk Clinical Commissioning Group which is in turn accountable to NHS England, and as part of Iceni Healthcare which are part of a local federated model. It is hoped that this will contribute towards shaping future health care in North Norfolk and benefit the patients of all the Practices concerned, whilst allowing individual Practices to maintain their characters and independence. Hospital Transport You may be eligible for hospital transport, however hospital transport is not provided for all, regrettably this decision no longer rests with the Doctor. Village Care Village Care co-ordinates a team of voluntary drivers who may be able to help with transport locally for medical appointments, this can be arranged by contacting Michael Gandy on the aforementioned number and e-mail. There is a box for donations towards the work of Village Care, a registered charity, on the front desk. More importantly, voluntary drivers are always required, as well as helpers for the Tuesday luncheon club which is also run and subsidised by Village Care. If you would like to offer help as a driver or helper, please speak to Michael Gandy. SystmOnline Aspects of your Summary Care Record (SCR) are available through SystmOnline which also provides electronic repeat prescription requests and appointment bookings / cancellations for the Doctors. The information available on the SCR is: - Current repeat medications - Acute medications - Allergies No other parts of your record are viewable. If you would like access to SystmOnline please ask at Dispensary, please note that photographic ID will need to be provided and a consent form completed before registering. Confidentiality Computerisation of medical records assists in health promotion and disease prevention, in monitoring repeat prescriptions and in dispensing medicines. Increasingly, information from local hospitals and NHS Norfolk is sent to the Practice via computer. Anonymous information held on computer may be used by authorised personnel for research. All records are entirely confidential and we aim to keep them fully up to date. All computer records are protected under the terms of the Data Protection Act. Electronic Data Sharing Aldborough Surgery operates Summary Care Record, Electronic Data Sharing and Care.Data initiatives, consent forms can be obtained from Reception or downloaded from our Practice website. Aldborough Surgery uploads SCRs daily and participates in the GP2GP records service which ensures timely and accurate electronic transfer of newly registered patient data. Comments and Suggestions If you have any comments or suggestions, or if you are unhappy with any aspect of the running of the Practice, please either speak or write to Dr Fleming or Mat Catley, General Manager. Complaints Procedure The Practice operates an ‘in house’ complaints procedure, details of which are displayed on the notice board or available from the General Manager upon request. If you are unhappy with the service you have received, it may be helpful to have an informal talk with the General Manager or Doctor in the first instance. Yes. Patients whose home addresses are outside of the Practice’s normal boundary area can still register at the Practice as an ‘out of area’ registered patient. How do I register as an ‘out of area’ registered patient? As an ‘out of area’ registered patient you are still required to complete the GMS1 and the new patient questionnaire, these can be found on our website at www.aldboroughSurgery.co.uk or alternatively you can collect the necessary forms from the Practice Reception. Once registered, you will still be offered a new patient appointment with a Doctor where clinically appropriate. What is the difference between a fully registered patient and an ‘out of area’ registered patient? The majority of services provided by the Practice, outlined in our Patient Charter, are available to both fully registered patients and ‘out of area’ registered patients. The only difference is that we are unable to provide in hours urgent medical care, which includes home visits to ‘out of area’ registered patients. In cases where these are deemed clinically appropriate this service will be provided by an alternative provider, suggested by the NHS East Anglia Area Team who will be responsible for ensuring you receive the care you require, we believe that it would be in your best interest and more practical for you to receive this service nearer to your home address. By calling 111 the Area Team will advise you on the nearest providers providing these services for you to choose from. How do I order and collect my repeat prescription and/or medication? If you live more than 1 mile from a Pharmacy your prescription and medication will be processed by the Practice. You can order your repeat prescriptions via SystmOnline, telephone, e-mail or face-to-face as outlined in our Practice Leaflet. If you live within a 1 mile radius from a Pharmacy your repeat prescription will be given to you to take to a Pharmacy of your choice. At present, you may receive your prescription in one of two ways, either collecting from the Practice or providing a stamped and addressed envelope so that we can post it to you.
