GENERAL PRACTICE RESIDENCY PROGRAM BROCHURE UNIVERSITY OF TOLEDO COLLEGE OF MEDICINE GENERAL PRACTICE RESIDENCY IN DENTISTRY The General Dentistry Postgraduate Program has as its primary goal the preparation of general practice which encompasses clinical, didactic, and research skills. The Advanced Education Program in General Dentistry is a 12month hospital based program and is designed to offer a balanced clinical and didactic curriculum in general dentistry. The faculty will provide a full range of clinical experiences in general dentistry which is supported by an in-depth, critical, and scholarly appraisal of the specialties knowledge base. Our mission is to provide each resident extensive clinical opportunities in behavior management, hospital dentistry, general anesthesia, pediatric medicine, emergency medicine, care of developmentally and medically compromised patients, trauma management, and other special areas. The clinical experiences are supported by a structured didactic seminar designed to encourage critical thinking appropriate to specialty level education in general dentistry. The overall curriculum is designed to prepare our graduates with the knowledge base and clinical skill level which allows successful entry into the contemporary practice setting of general dentistry. The program has been planned in accordance with the guidelines for postdoctoral education of the American Dental Association Council on Dental Accreditation. Upon completion, the resident receives a certificate in General Dentistry. The program is accredited by the Commission on Dental Accreditation. Program Description The Division of Dentistry offers a residency program for postdoctoral dentists who seek additional training in general dentistry. The development of skill in clinical general dentistry is the primary program objective. Curriculum The program involves clinical exposure and didactic coursework that provides the resident the educational activities recommended by the American Dental Association Commission on Dental Accreditation. The didactic curriculum consists of a series of interrelated seminars about basic general dental care, care of the handicapped and chronically ill, and diagnosis and treatment of occlusal problems in primary, mixed and adult dentitions. The residents will complete clinical rotations in the Emergency Room, Anesthesiology Surgery and Pediatric Medicine. Admission Requirements Completed application Three letters of recommendation (one must be from the Dean or his/her designee) Official Dental School Transcripts Personal interview Goals and Objectives The training program has been developed within approved hospital dental services by nationally promulgated American Dental Association standards. The Dental Service is approved by the Council on Hospital Dental Service of the American Dental Association. The goals of the University of Toledo General Practice Residency Program is to prepare each resident to: 1. To act as a primary care provider for individuals and groups of patients. To achieve this goal the resident provides emergency care in the emergency room of the hospital and in the dental service on the 2nd floor of the outpatient building. To provide comprehensive oral care the resident are involved in the examination of each patient and help in the treatment planning process. The treatment planning conference is held each week from 12:00 p.m. to 1:00 p.m. in the ENT/Dental conference room. Each patient is reviewed from the medical standpoint and the dental standpoint. They have had a full series of x-rays taken and mounted study casts. At the consultation appointment the resident discusses disease prevention activities and necessary care with each patient. The necessary advanced dental treatment will be prepared in the OR or Dental and patient setting. 2. To plan and provide multidisciplinary comprehensive oral health care for a wide variety of patients including patients with special needs. To achieve this goal the residents work with patients and their referring physicians from the Northwest Ohio Developmental Center, regular and emergency patients from the dental service at the Medical College, patients from the homeless VA program, and patients with special needs. Patients are also referred to the dental service from the solid organ transplant center, radiation therapy, and the HIV special clinic. A resident and attending plan and carry out the necessary care in the OR or outpatient dental service. 3. To manage the delivery of oral health care by applying concepts of patient and practice management and quality improvement that is responsive to a dynamic health care environment. To achieve this goal residents participate in a weekly conference with the dental staff to manage the practice as a team within the dental service. A quality improvement format is used to respond to the changing needs of the dental service and hospital environment. Practice management methodology has been based on the teachings of the Pankey Institute. The textbook used for discussion is, “A Philosophy of the Practice of Dentistry,” by L.D. Pankey. 4. To function effectively within the hospital and other health care environments. To achieve this goal residents are appointed to the hospital as house officers and do rotations in the emergency room, anesthesiology, and ENT. 5. To function effectively within interdisciplinary health care teams. To achieve this goal residents along with attendings participate in a wide variety of interdisciplinary health care teams to provide care for patients within the hospital. They answer consults in the dental service and on the floors of the Medical College of Ohio hospital. They work with other residents and attendings on their rotations in the ER, Anesthesiology and Medicine. 6. To apply scientific principles to learning and oral health care. This includes using critical thinking, evidence or outcomes-based clinical decision-making and technology based information retrieval systems. To achieve this goal, the residents participate in an outcomes measurement tool developed by the division of dentistry. Critical thinking is developed during the weekly journal club, grand rounds, and treatment planning sessions. Each resident has access to a Dental/ENT library and the Mulford Library, which are both connected to the internet. The library gives the residents the use of technology based information systems. 7. To utilize the values of professional ethics, lifelong learning, patient centered care, adaptability, and acceptance of cultural diversity in professional practice. To achieve this goal residents participate in a weekly journal club and grand rounds in which they will discuss reviews of current literature in the areas of professional ethics and specialized patient care. In addition, during hospital orientation the residents receive training from the Associate Dean for Clinical Affairs and Medical Director, in Ethics, Pain Management, Computer Order Entry, Lab Viewer, Inpatient Lab, Cost Containment and Resource Utilization. 8. To understand the oral health care needs of communities and engage in community service. To achieve this goal the residents participate in an annual table clinic meeting with the Toledo Dental Society. Also upon request during the year, the residents go to local schools and provide teaching of oral health care procedures to grade school students. The pediatric dentistry faculty provides information and education on health care needs to the community. ROTATION OBJECTIVES It is important for the dental residents to be able to understand the interrelationship of medicine and dentistry in order to continually provide high quality comprehensive dental care for their patients. The dental resident must be able to develop a good working relationship with other professionals in the hospital setting. In the final analysis this aspect of the residency may be the most important in the learning process geared toward providing your patients with the best possible care. This material has been organized according to the overall goals of the American Dental Association and the Medical College of Ohio, Graduate Dental Education Program. Dental Service In order to meet The Standards for Advanced Education Programs in a General Practice Residency Program the following guidelines will be met: A. When presented with a patient in the dental service, the resident will demonstrate comprehensive dental care in the following manner: Review medical and dental histories. Perform oral examination and cancer examination. Mount cast to articulator. Plan a detailed course of dental treatment for the patient. Evaluate findings and consult with the patient about the results of the examination. B. When presented with a patient in the dental service, the resident will demonstrate proficiency in skills and techniques related to: Endodontics Periodontics Oral Surgery Restorative dentistry (to include fixed and removable prosthodontics) Pain Control Methods of behavior modification and stress management needed when providing dental care to children and adults. Completion of dental outpatient charts C. Resident will demonstrate competency in evaluating and managing dental emergencies, trauma and acute oral pathological conditions Davis/Kaechele Rotation When presented with a patient on this rotation, the resident will demonstrate proficiency in skills and techniques related to: Admitting and discharging patients Obtaining medical history and physical examination with the attending physician Prescribing pre-operative and post-operative medications with emphasis on the actions, interactions, indications, contraindications, and toxic reactions of those drugs. Methods of handling tissue to minimize trauma The initiation, development, and completion of the patient's chart, including: - Soap notes - Writing progress notes - operative reports - discharge summaries Assisting in the performance of surgical procedures Post-operative care and evaluation of patients including: - Changing dressings - Prescribing appropriate medications for pain and infection control under the direction of the ENT attending treating patients with multiple medical problems and severe communication deficits function in multiple institutional settings, hospital, nursing home, institution for physically and mentally handicap patients communicating with physicians and other medical professionals regarding patient care managing in-patient and out-patient surgery under general anesthesia treating patients with a blood/bone diseases (Hepatitis, HIV) having exposure to other disciplines including Radiation Therapy and Speech Therapy A. Residents must be able to demonstrate competency in requesting and responding to requests for consultations from physicians and other health care providers B. Residents must be able to demonstrate competency in the management of pain and anxiety in delivering outpatient care using behavioral and pharmacological modalities beyond local anesthesia C. Residents must be able to demonstrate competency in physical evaluation and medical risk assessment, including: taking, recording and interpreting a complete medical history understanding the indications of, and interpretations of laboratory studies and other techniques used in the diagnosis of oral and systematic diseases understanding the relationship between oral health care and system diseases interpreting the physical evaluation performed by a physician with an understanding of the process, terms and techniques employed using the techniques of physical examination D. Residents must be able to demonstrate competency in understanding of hospital organization, functioning and credentialing process. E. Residents must be able to demonstrate competency in the management of inpatients or same-day surgery patients, including: admitting and discharging patients obtaining medical histories and conducting appropriate physical examinations prescribing treatment and medication providing care in the operating room preparing the patient record, including notation of medical history, physical examination, pre- and post-operative orders, description of surgical procedures and discharge summaries F. Residents must be able to demonstrate competency in the discussion of patient diagnosis, treatment planning, and progress and outcomes of treatment. G. Residents must be able to demonstrate competency in reviewing relevant scientific literature. Emergency Room Rotation When presented with a patient on the emergency room rotation, the resident will demonstrate proficiency in skills and techniques related to: anticipating, evaluating and managing systematic emergencies. review medical histories to recognize factors which may have predisposed a patient to systematic emergency. help support respiration and/or circulation when a patient collapses. evaluate and manage a patient with a seizure or sudden loss of consciousness. help treat a patient who was suffering from an allergic reaction. assist the emergency room attending and staff with the care of patients in the emergency department. All residents are certified in basic life support and advanced cardiac life support. Residents also must have experience in the diagnosis and treatment of emergency problems related to the oral cavity. They have skill and knowledge to diagnose and to provide primary care for acute infection of oral origin, acute disorders of the oral mucosa, hemorrhage of the oral cavity, and traumatic injuries to the dental and maxillofacial structures. Specific skills that residents acquire in order to diagnose and treat dental emergencies include: Dental patient assessment (e.g., specification of needed radiographic and laboratory data) Wound debridement Treatment of tooth fractures Anesthesiology Rotation When presented with a patient in the anesthesiology rotation, the resident will demonstrate proficiency in skills and techniques related to: Assist in the preoperative evaluation of patients before surgery. Assess the effects of pharmacologic agents on patients. Assist in venipunture techniques and administered intervenous agents. Monitored patients during general anesthesia. Help manage the airway of conscious and unconscious patients. Assist during anesthetic induction and intubation. Give anesthetic agents. Explain the theory of prevention and treatment of anesthetic emergencies. Assist in the management of patients in the recovery room.
