Unique Patients Served in Primary Care • 2009 - 1,561 • 2010- 2,081 • 2011- 2,927 • 2012- 4,259 • 2010 Acquire the Site from FMOL and operate as is- 4 Physicians, 3 Nurse Practitioners, Lab and Radiology Services • 2012 Cross over the Center to an FQHC, staff still the same, some services dropped due to new Hospital opening, GNOCHC program accepted, still in trailers. • 2013 Jan-Feb preparations to move into new location in MOB of Hospital. Plan for 2 Physicians, 3 Nurse Practitioners, Behavioral Health Providers, WIC and St Bernard Parish Sanitarians at site. Unique Patients Served at Site • 2011- 12,847 in Primary Care, 1,183/ GNOCHC • 2012- 12,204 in Primary Care, 1,891/ GNOCHC • 2012 1st FQHC Opens in Plaquemines Parish at Belle Chasse. Currently housed in space formerly occupied by Coroner of the Parish who now works as a Family Practice Physician for the CHC. A Pediatrician is also practicing at the site. Collaboration efforts are underway to solidify the clinic as well as grow its services into the near future. Lessons Learned • Plan-Plan-Plan! Strategy • Pick your Partners Carefully.
