Recordable Injury
It was during the last
descent down the
structure that a team
member caught his
right hand between the
elevator doors, as he
was attempting to close
the elevator and place
it back into service.

 Remember, all
elevators are not
built the same.
 Elevator features
and operation may
vary from one
elevator to another.
Inspection
 Employees will no longer be
allowed to operate an
elevator without the approval
of their supervisor.
 Supervisor will designate an
employee to operate the
elevator.
 Short service workers are not
allowed to operate elevator.
 Supervisor and operator will
do a thorough inspection
prior to use.
Elevator Pre shift Inspection
Checklist
Ask yourself the following questions to help check that
the elevator is approved for use.





Do elevator doors have attached straps or handles?
Control buttons appear in place?
Does the elevator state the maximum load capacity?
Are you authorized to operate the elevator?
Did you answer "YES" to the above questions?
A “no” answer to any above item equals a safety concern.
Contact your supervisor before using the elevator.
Authorized Elevator Operator
 Operator will be the
only person to close
and open the doors.
 Operator will not
allow anyone to help
or assist him when
handling the doors.

“Must use the nylon straps to open or close the doors”.

"Never open elevator doors while elevator is moving, wait until
the Elevator comes to a complete stop before opening the doors".

 Don’t over load the
elevator.
 Obey the Elevators

capacity.
 Don’t allow too
many people inside
the elevator.

 Power failure.
 Malfunction of
control components.
 Activation of safety
equipment.
