Volunteer Applications are now being completed online by your Field
Instruction Program Lead, follow the guideline specified provided to you
• The Volunteer Application and TB Clearance need to be faxed o SMH Field
Instruction Office , attn: Maria Hu Cordova 213 241-3305
• FI Program Lead will provide the finger print appointment to the intern via email.
Fingerprint appointments are being scheduled the week of Sept 10th and the
week of the 17th – .
• If your background has not been cleared by employee relations then you will be
supervised by the field instructor at all times. You are not to be left alone with
any student until your background has been cleared.
