LGH provides a full range of general and many specialized acute care services . The hospital has 268 beds, 7 operating rooms, and a variety of diagnostic services. It is the 4th busiest hospital in Vancouver and 1 of only 5 neurosurgery centres in B.C. Change Ideas List Changes you have tested during Falls VLC PDSA Cycles: 1. Implement double-sided non-skid socks on all units for those who do not have appropriate footwear. 2. Provide education on completing CAMP (the risk screening tool) on the assessment form for all patients on 4 east, 4 west and 7 east. 3. Revise fall prevention preprinted customizable care plan and implement on 4 east, 4 west, and 7 east. Care plan to be completed for those patients that score 2 or more on the CAMP at admission. 4. Implement “falling man” decal as visual identifier for those at risk of falls and with a fall prevention care plan. Lessons Learned/Key Insights - Regular meetings were very important for keeping us on track - It is necessary to have a key person to lead the changes on each unit. - Regular ongoing education on fall prevention and PDSA focus - Ongoing regular updates to the staff on progress and data - Involve the frontline staff!!!
