UK-HMO and UK-RHP copayment increases high end diagnostic and hospital observation by $25 per visit. 3 Health Plan Premiums Health plan premiums will increase 2% overall and employees who receive the health credit will see a $1-$7 increase per month for UK-HMO, UK-PPO and UK-RHP plans. UK EPO premiums will increase between $5-$14 per month. The overall health plan costs are increasing by only 2%. The university is contributing an additional $2.1M to cover the increased cost for FY 2014-15. 4 UK-RHP Provider Regions have been expanded, increasing your options when choosing a doctor. If you reside in Region A, you can now see any provider located in Region A. (see map) If you reside in Region B, you can now see any provider located in Region B. (see map) 5 Region A 6 Dental Plans Delta Dental premiums will remain unchanged and UK Dental premiums will increase between 1 and 10%. Vision Plan premiums will remain unchanged. You may now roll over up to $500 from your Health Care FSA left unused to the following plan year. You will still have the grace period (9/15/2014) to incur claims for FY 2013-14 and you can file claims through 12/31/14.
