FARADISM UNDER PRESSURE
Aim: To reduce swelling & oedema.
Pre requisite - Part should have intact nerve supply.
Principle
1) Gravity
The limb is elevated at angle of 45° with help of pillows to allow fluid drainage.
2) Compression
Rubber bandage provides compression forces due to stretch and elastic recoil during muscle
contraction that facilitates drainage.
3) Voluntary contraction
SSF produces muscle pumping action which helps to drain fluid.
For Lower Limb
→ Patient should be supine with limb to be drained, elevated using pillows.
Preparation
→ Rubber tourniquet / elastocrepe bandage is used such that the roll faces the therapist
→ Two folds are made on foot leaving toes out
→ Then criss cross pattern in figure of 8 is made
→ Pressure of bandage should reduce as we go proximally
Electrode Placement
Active: Gastrocnemius belly
Passive : Heel region on the sole of the foot
→ No air pockets should be there, if so, fill with cotton/gauze
→ Pattern of wrapping should be uniform with equal overlapping
→ There should be no numbness sensation in limb
→ Check for temperature & capillary refill in toes
Current: SSF
Duration: 15–20 min
→ Post treatment check skin for abnormalities
Upper Limb
Position
Supine with limb elevated above heart level to facilitate drainage in axillary lymph nodes
Therapist Preparation
A rubber tourniquet is used. Due to property of elastic recoil it provides pumping action to assist
fluid drainage.
2 folds are made in region of palm leaving thumb and distal phalanx to check for blood circulation.
After that criss cross pattern in figure of 8 upto the joint where contraction is required.
Placement of Electrodes
→ Active electrode at flexor aspect of forearm at junction of proximal 1/3rd and distal 2/3rd
→ Passive electrode in the palm of hand
Precaution
→ No air pocket should be formed to avoid accumulation of oedematous fluid in uneven area
→ Patient should be asked to report in case of numbness or tingling sensation
→ In case of marked discolouration treatment should be stopped
