SPS 252 Spring 2024-2025 IEP Assignment Task: Develop an IEP for one child of focus for a duration of 3 months and for the following areas: Language Arts, mathematics, and a 3rd area your choice. The assignment should include the following sections: 1. Student Information: Briefly describe the child’s background - Student Information Student Name (Initials) - Student Date of Birth/Age - Grade Level - ** Diagnosis - **Profile (General relevant information including educational concerns and learning strengths and challenges) 2. Present Levels of Academic Achievement & Functional Performance (PLAAFP): Provide an overview of the child’s current performance in each focus area, supported by observations, assessments, or other relevant data (source of data & sample evidence) 3. Measurable Goals: Define goals for each area of focus Include short-term objectives and/or benchmarks that break down each goal into smaller, measurable steps. Provide projected dates for achieving each short-term objective and benchmark. 4. Services: Identify the special education and related services that will be provided to support the child’s learning in each area. This may include specially designed instruction, related services, or any other necessary support. Clearly delineate how these services will be delivered (for each service, include Location, Frequency, Anticipated Duration, & Service Provider, as applicable) Include Accommodations and/or modifications 5. Measuring & Reporting Progress: Explain how the child’s progress will be tracked, including the tools or methods (e.g., observations, formative assessments, work samples) used to measure growth. Specify the timeline for progress reviews and assessments (e.g., weekly, biweekly, monthly). Also specify when periodic reports on progress will be provided. 6. Participants: List the key individuals involved in the development and implementation of the IEP. Format: You may present the above components as text with the above headings or in tabulated format. Note however, that tabulated formats allow quick reference and clarity, making it easier for teachers and parents to find key information.
