FLOW CHART: Lock-Out/Tag-Out Procedure
Stop/Shut down equipment.
Identify each energy source.
Isolate each source of energy with
suitable Lock-Out devices.
Lock-Out and Tag-Out by each person.
Release stored energy if any.
Verify effectiveness of
Lock-Out/Tag-Out.
Perform required job.
Remove Lock-Out/Tag-Out.
Lock-Out/Tag-Out Removal Procedure:
Inspect work area (Machine guards in place, tools
removed, all persons are in safe location).
Remove locks and tags
and isolating device.
Re-energize the equipment.
