At the end of this learning activity, the FS students should be able to:
1. identify the school forms, its purpose and uses;
2. download and fill-out sample DepEd school forms; and
3. do sample computation of grades using the DepEd assessment criteria.
The K-12 Grading System, Electronic Class Record Templates, and DepEd Forms
. The K to 12 Basic Education Program uses standards- and competency-based grading system. When we say
grading system we mean a system that educators use in a particular institution to evaluate each individual student’s
performance and learning. Grading can include letter grades, percentages and even a simple pass/fail. Grades can be
attached to physical activities like writing assignments, lab work, projects, reports and tests. In DepEd, all grades are
based on the weighted raw scores of the learners summative assessments. These summative assessment can be
classified into three components, namely, written works (WW), performance task (PT), and quarterly assessment These
three will be the bases for grading. The nature of the learning area defines the way these three components are assessed.
For Kindergarten, checklist and anecdotal records are used instead of numerical grades. These are based on
learning standards found in the Kindergarten curriculum guide. For grades 1 to 12, numerical grades are used. The
minimum grade needed to pass a specific learner’s area is 60, which is transmuted to 75 in the report card. The lowest
rank mark that can appear on the report card is 60 for quarterly grades and final grades.
Teachers in the Basic Education implementing the K to 12 program uses electronic class record in excel format to
facilitate high level of accuracy in the computation of the learners grade. Each learning area has its specific e-class record.
Aside from these, different DepEd forms are accomplished at varied time within a school year.
In this activity, FS 2 students will be exposed to various DepEd forms and e-class records. They will be given
opportunities to examine and accomplish different forms, compute quarterly grades and final grades, and accomplish eclass records aligned with their specialization.

Answer the following questions:
1. Which is easier to do, manually compute the grade of the students or the use of e-class record? Why?
➢ It is so much easier to use e-class record because of the following reasons:
a. Grades will automatically be computed by inputting scores and formulas.
b. It saves time.
c. It lessens the possibility to compute grades incorrectly.
d. One click, one grade.
e. You can make adjustment as long as you want.
f. It doesn’t necessarily need a lot of procedures to compute a grade.
2. Is it important to employ a systematic approach in computing the grades of students? Justify your answer.
➢ Grades are basis in evaluating student’s performance and behavior that is why it should be computed accurately and
systematically. Additionally, here are some roles of grades.
a. Means of communicating with students, parents, and other school stakeholders before going to college and for further success.
b. Grades can be a source of motivation for students.
c. Lesson will be organized as grades mark transition to another course.
d. Students can have feedback about their learning.
e. Provides self-evaluation for teachers.
3. Why is there a need to accomplish the different school forms?
➢ At the end of the School Year, one of the most significant activities in schools is the creation, production and checking of
all school forms which is done to verify the quality and consistency of learner information. Additionally, these school forms
were tedious and took up a significant amount of time and effort on the part of teachers as it is prior to the issuance of
DepEd Order No. 4, s. 2014 or the Adoption of the Modified School Forms for Public Elementary and Secondary Schools
Effective End of School Year.
4. When are the different school forms need to be accomplished? Why?
➢ Forms are put on different schedules or time frames that encompass report upon the semester or a school year, and the
time recommended for forms needed varies based on what is categorized. For instances, the SF1 must include the
number of students enrolled at the start of the semester, whereas the SF5 must be completed at the conclusion of the
school year when they are evaluated.
5. What will happen if the school forms are not accomplished on the specified schedule?
➢ These documents are necessary in most cases, just as having an inventory of store supplies is. If this happens, chaos will
reign, and school requirements will be seriously harmed, compromising the institution’s ability to function. Furthermore,
because most of these school forms are interrelated, various school processes will be jeopardized, resulting in a cascade
of problems.

Answer the following questions:
1. If you are the teacher, what approach will you use to systematically compute the grades of you students? Why do you
prefer that approach.
➢ I would rather use the using of Microsoft Excel and other application to compute my students’ grades
accurately and automatically. Additionally, this would help me to reduce possible incorrectness of grades.
Sometimes, manually computing the grades will give you a higher possibility to compute it incorrectly.
Moreover, there are some reasons why I prefer to use digital application rather that doing it manually.

2. Which of the different school forms is the easiest to accomplish? Why?
➢ SF1 requires minimal information among students. It has information such as, name of the students, address
and other basic personal information of the students that is why; it is the easiest school form to accomplish.
3. Which of the different school forms is the most difficult to accomplish? What will you do to lessen the difficulty
encountered in accomplishing this form?
➢ The SF 9 or the Learner’s Progress Report Card, with its other components and partial complications will be
the hardest to complete. It comprises a quarterly report on a learners’ academic, behavioral and attendance
progress. The most difficult part there is the calculation of each student’s grade individually, and such an
exercise cannot be completed without making a mistake and having to go through everything again. All you
must do to make this form easier to accomplish is to be careful and patient, which takes practice.
