NA PROCEDURE 1.0 Responsibility 1.1 1.2 1.3 It is the responsibility of all employees to report near misses. It is the responsibility of the Health and Safety Advisor to monitor investigations. It is the responsibility of the relevant manager to carry out investigations with appropriate personnel. 2.0 Definition of a near miss or incident ‘ A near miss is an event that narrowly missed causing injury or damage’ 3.0 How to use the near miss incident report forms. 1. The initiator raises the near miss report using the form shown below, copies are held in the Incident Report sign. Completed forms are handed to the line manager for submission to the Health & Safety Advisor. 2. The Health & Safety Advisor will log the near miss / incident report on the register. 3. The relevant manager will carry out an investigation and agree any corrective action with the relevant department managers and operatives. 4. Agreed corrective actions are assigned to specific competent personnel with agreed completion dates. 5. The status of corrective actions are reviewed by the Health and Safety Advisor on an agreed time scale, urgency dictated by the seriousness of the incident. 6. After the completion of the actions associated with the near miss / incident, the Near Miss report is closed out and feedback given to the department manager and initiator. 7. The incident report log is updated. 8.
