Ground up 100,000 square foot medical office, that will contain a pharmacy, dental adult and
pediatric medical care. With office space, classroom, physical therapy, and drive through
pharmacy.
Scope of work:
Clearing and grubbing, over excavation, foundation, structural steel, concrete slab, exterior and
interior framing, and finishes, underground water, sewage, power, site concrete, asphalt, and
landscaping.

DETAILS
A project trailer is onsite.
The project is fenced in, security after work
hrs and weekends, and signage is in place.
The company safety manual in on the server,
and a copy is printed for workers to look at.
All safety posters are in the trailer.
The emergency action and crisis plan is
located at the back of the JSSP.
On-Site Health and Safety will be called for
all PENTA injuries and drug testing, if the
injured worked need additional treatment
they will be taken to Concentra Medical
Center located at 5850 Polaris Ave. Las
Vegas NV 89118 The subcontractors can use
any medical center they like.
The OSHA Notification Letter was turned
into OSHA on 12-09-2022
The report has been sent to the project
superintendent.
Dust permit has been completed and the dust
control sign has been installed.
SWPPPS has been completed and
subcontracted to a third party.
Any work containing heat, flame sparks a hot
work permit must be completed and signed
by a PENTA supervision before work can
began.

All PENTA workers will attend a weekly
toolbox safety meeting, each subcontractor
will have their own toolbox safety meetings.
Before any worker begins workers are
required to attend their company daily
huddle.
Before any worker is allowed to work onsite,
they are required to attend a site safety
orientation, complete the orientation sheet
and quiz.
When a worker goes over and beyond safety,
they are given a safety champion card that
they fill out and return for a t-shirt, their card
will go in a monthly drawing.
Weekly OAC, subcontractor, and site wide
safety meetings are held.
Each subcontractor will attend a Premobilization meeting. We will discuss their
site JSSP, JHA and chemical inventory.
All workers will have a minimum of a
OSHA 10hour and all workers that direct
work will have an OSHA 30hour.
PENTA supervision will be First Aid/CPR
trained, each subcontractor will have their
own training conducted.
Before any crane arrives onsite, a crane plan
must be turned in for review, and it may be
sent to a third party for review. When the
crane arrives onsite a copy of the annual cert
and operators’ certificate will placed in their
folder.

INTRODUCTION
This Crisis Management Plan is written to provide specific actions to be taken during and
after a crisis. Since many of the actions outlined in this plan are to be taken after a crisis has been
resolved, you must follow the Crisis Event Plan (CEP) located on the back side of each sitespecific Emergency Action Plan (EAP) for all immediate actions to be taken during the
crisis.
Since many crisis events do receive media coverage; John Cannito, Joel Wallis, Mitch
Bernard or Tony Cornell should deal with the media. If you are faced with any media questions
please say the following company statement; “We are aware of the situation and are
investigating the details. We will keep you informed as the situation progresses. The safety
and well being of our employees is our highest priority.”

Gather all available information.
Who, What, Where, When, How, Etc…? Hospital info/phone #’s.
(Only John or Joel deal with the media and next of kin)
Initiate discussion with VP of Field Ops regarding job site closure. VP of Field Ops will
determine if closure is necessary.
Initiate discussion with Vice President of Operations re: media spokesperson. John or
Joel
Notify jobsite receptionist route all calls to the designated on-site person for screening.
All messages should be forwarded to John or Joel.
Jobsite receptionist must also notify Sandy at the main office so she can notify all of the
other jobsites.
Instruct all PENTA salaried, craft and Sub employees to refer any questions to the
designated spokesperson. John or Joel.
Initiate investigation process with assigned team and begin to prepare reports (See
Accident Investigation Process in Safety Manual). All reports directed to John or Joel
should be marked "Privileged and Confidential."


Notify President, V.P. of Operations and V.P. of Finance

Inform all on-site employees (subs & PENTA) to refer all media calls to John or Joel

Inform the project owner(s) or representatives of the event(s).

Proceed to the incident site as soon as possible to assist PM and/or Site Safety
Manager(s) in interviewing witnesses, gathering evidence, providing guidance,
contacting 3rd party agencies (MFG’s reps, rental companies, etc.)

Inform President of all developments as the crisis unfolds.

Assist with reports and corrective/preventative actions with job team.

PENTA project staff will take charge of all initial emergency actions and crisis management requirements.
PENTA staff will do whatever is necessary to maintain control of the project during the emergency.
Subcontractor will be instructed to comply with all PENTA requirements and requests during the emergency phase of the
crisis.
Subcontractor should not take any actions without first consulting PENTA management. This includes media contact.
PENTA upper management will decide at what point further actions will be referred to the involved subcontractor.
PENTA will continue to assist as determined by project management. This should be dealt with on a per project basis

REFER ALL INQUIRIES, ESPECIALLY MEDIA, TO THE CORPORATE SPOKESPERSON. John, Joel, Mitch or

Project personnel are NOT to speak with the media.
Always say the company statement; “We are aware of the situation and are investigating the details. We will keep you
informed as the situation progresses, but know that the safety and well being of our employees is our highest
priority.”
Speak privately with officials; never speak in front of media or public.
Sr. MGMT will determine the extent of PENTA’s involvement w/ media.
PENTA may speak for a subcontractor in cases where representation is onsite or where subs may want to defer to PENTA.
This will be decided on a case by case basis, by Sr. MGMT only.

Make sure the area is safe from further possible injuries.
Clear the immediate area and stop all work. Attempt to preserve the accident site as much as possible.
Stay with the injured person to monitor any changes in their condition and help keep them calm.

Proceed to assigned area; emergency vehicle entry point, accident scene, etc.
Ensure site access roads are clear for ambulance to enter.
Escort emergency vehicles & personnel on to the site and to the injured employee.
Maintain a safety zone around the general area of the injured party and the incident scene.
Protect all equipment, machinery and material involved in the incident, for further investigation.
IMPORTANT!!!:



During the emergency, all work in the immediate area of the incident scene and at all access routes shall be stopped.
All co-workers shall leave the immediate incident scene, unless they are required to assist the emergency workers.
All co-workers must follow the EMERGENCY EVACUATION PLAN if the incident is of a CRISIS EVENT
nature.
EMERGENCY EVACUATION PLAN & SITE MAP
In case of a CRISIS EVENT requiring the evacuation of the jobsite, follow the CRISIS EVENT PLAN on the back.
 Evacuation notification will be made by; 5 blasts of the crane horn, radio, cell phone or verbally.
 Each company is responsible to account for all their co-workers and report to PENTA supervision.
 All co-workers shall proceed to the evacuation assembly area:
 Primary = Primary Gate #1 on Craig Ranch View Court
 Secondary = Secondary Gate #2 Craig Road
 DO NOT leave the jobsite unless you are accounted for and have been instructed to do so by your supervisor.

STEP 3: CONTROL
MAN THE GATES TO CONTROL ACCESS / EGRESS OF THE SITE
o Assign PENTA employees to man specific gates to insure that emergency personnel
are properly directed and that visitors (including media) are NOT allowed on site.
o Do NOT allow workers to leave the site until the situation has been assessed.
STEP 4: COMMUNICATE
CALL EMERGENCY MEETING W/ PENTA & SUB SUPERVISORS REGARDING SITUATION
o IF NECESSARY, activate emergency notification system (5 crane horn blasts, two-way
radios, cell phones or verbally)
o Gather & account for all employees in the EMERGENCY EVACUATION ASSEMBLY AREA
o Tell them that we are handling the situation and if media personnel arrive on site to
say, “WE’RE AWARE OF THE SITUATION AND ARE INVESTIGATING THE
DETAILS. WE WILL KEEP YOU INFORMED AS THE SITUATION
PROGRESSES”.
STEP 5: COORDINATE
INSTRUCT PERSONNEL AS TO WHAT DO NEXT
o After emergency personnel have control of the situation join the employees at the
emergency evacuation assembly area (if applicable) to explain the situation and
coordinate who should leave or return to work.
o Initiate the CRISIS MANAGEMENT PLAN; Gather any witnesses, begin individual
interviews, assign someone to take pictures or video of the scene, collect any
evidence, etc.
