Greetings!
I am Reynaldo Villamayor Sales, of legal age, married Filipino, the owner of Rey and Nedy Motorcycle
Spare Parts and Accessories for almost six (6) years located at San Roque Poblacion, Nabua, Camarines
Sur. DTI Permit- expired last december 15, 2021.
Amend to this, I would like to request to your good office to consider the renewal of my Business Name.
The reason for the unavailability to process the renewal was because I decided to sell and find a
potential buyer of my business due to lack of sales brought by the Covid19 pandemic, but we still have a
permits to operate.
To support my statement, I have attached a legitimate proof that the business is operating and is using
the business name for business related transactions such as, Certificate of Registration from the BIR,
Official Receipt, Previous DTI-Permit and Mayor's Permit.
