TITLE:
Gross Urine Procedure
I.
Purpose
This procedure addresses any urine sample that will yield erroneous result on
chemistry dipstick that will inadvertently clogged the sample probe, block aperture
flow and filter and interfere with the visual detection of pertinent microscopic
elements due to grossly bloody urine, excessively mucoidy, viscous, turbid, and
pasty/sticky samples.
II.
Procedure
If you have grossly bloody urine (red opaque), dark turbid orange, extremely
mucoidy, viscous, turbid, and pasty/sticky samples
Steps are as follows:
1. Order urine microscopic in Cerner by adding on to the existing accession #.
2. Cancel the UA screen with the footnote based on the integrity, color or
volume with the following canned comments:

ublood (unable to give accurate urine dipstick results)

uorg ( Urine is orange; unable to give accurate urine dipstick results.
Phenazopyridine or Rifampicin causes urine to turn orange that may mask
or mimic positive reactions on urine chemistry strip, which yield
erroneous result.)

uvol ( Specimen received with volume less than 5ml; microscopic only
performed.)

uascant ( Specimen received with volume of less than 2 ml; unspun urine
microscopic only performed. Interpret with caution. Result may not be an
accurate reflection of patient status due to less recovery of cellular
elements on low sample volume compared to at least 5 ml of urine sample)

Unacceptable specimen – refer below
 Urine chemistry testing is not possible due to sample integrity - extremely
turbid, viscous, mucoid, or pasty that may clogged sample probe and filter
causing interference or carryover onto succeeding urine sample.
 Cancel macroscopic test (urine chemistry strip) and footnote “Sample
extremely turbid, viscous or pasty. This can clog sample probe and may
yield reading error in chemistry pads of the strip”.
 Then perform manual microscopic analysis of the urine. Do not load urine
sample in iQ 200. Copy and paste the footnote text on WBC result
comment so that Physicians can still see the reason why Urinalysis
chemistry result was not performed.
3. Transfer the urine from MOD Iris to MOD Manual urinalysis:
a. Go to pending inquiry
b. In test site enter MOD Iris
c. Select the test
d. In task (Upper left corner) ---Transfer---Select order---All Details.
e. You will see a pop-up window that reads:
i. Transfer all orders to:
ii. MOD Manual Urinalysis
iii. The download to instrument will automatically check OK.
4. Results in ARE, drop down ranges will be available for reporting.
Ensure the transfer is instrument to instrument. It cannot be from super group MOD
Urinalysis SS.
