PERFORMANCE MONITORING AND COACHING FORM DATE CRITICAL INCIDENCE DESCRIPTION Conducted a coaching session to analyze student writing samples and provide and provide feedback on language proficiency and writing skills. Conducted a collaborative meeting to review student speaking and listening skills using learner feedback and assessment data. Facilitated a mentoring session to analyze student work samples and provide constructive feedback. Discussed effective instructional strategies to deepen conceptual understanding and problem-solving skills in math. Organized a Focus Group Discussion (FGD) to share and reflect on effective instructional strategies and resources. Collaboratively planned activities to enhance students’ understanding of moral dilemmas and ethical decision making. OUTPUT Implemented targeted interventions and provided individualized support resulting in improved writing skills and language proficiency in Filipino. Implemented speaking and listening activities resulting in improved communication skills and language fluency among students in English. Implemented instructional strategies resulting in improved student performance and confidence in math.
