dealer/workshop.
Photographs of the damaged vehicle/property showing number plate and
damages distinctly.
Statement of driver on the wheel at the time of accident and copy of his valid
driving license book.
Notification of the loss to police & their report.
Copy of whole set of valid registration book of the damaged vehicle.
Copy of Route permit & Janch Pass of the vehicle valid at the time of
accident.
Load chalan /Passenger chalan of the vehicle related to the time of accident.
Bill & Cash Memo related to the repair and replacement of the damaged
vehicle/property.
Other claims related documents required under merit basis of loss.
In case of Third Party /Passenger/Staff‘s Bodily Injury


Complete sets of medical papers including doctor’s prescriptions, Medical
cash memos, Pathological reports, Admission & Discharge Ticket & other
treatment related medical documents.
Aattested copy of Milapatra and all other claims related documents as
needed.









Letter issued from CDO office to release compensation.
