We, grade 12 STEM students from the University of the East – Caloocan Philippines are currently
doing study entitled, “Impact of the COVID-19 Quarantine to the Nutritional Status of Grade 12
Students”.
Our study aims to investigate the impact of the COVID-19 quarantine on the nutritional status of
Grade 12 students of the University of the East under the Academic Track by analyzing before and
during the quarantine period, the changes in their body mass index, stress levels, dietary habits, and
physical activity. In this connection, we would like to seek your permission to use the table 2: Answers to
the Questions of the Questionnaire in your study entitled “Changes in Weight and Nutritional Habits in
Adults with Obesity during the “Lockdown” Period Caused by the COVID-19 Virus Emergency”.
