ACTION
RATIONALE
1. Upon arriving at the clients home, place the bag
on the table or any flat surface lined with paper
lining, clean side out (folded part touching the
table). Put bags handle or strap beneath the bag.
To protect the bag from contamination
2. Ask for a basin of water and a glass of water if
faucet is not available. Place these outside the
work area.
To be used for hand washing and to protect the
work field from being wet
3. Open the bag, take the linen/plastic lining
spread over work field or area. The paper lining,
clean side out folder part out.
To make a non-contaminated work field or area
4. Take out hand towel, soap dish and apron and
place them at one corner of the work area within
the confines of the linen/plastic lining.
To prepare for hand washing
5. Do hand washing. Wipe dry with towel. Leave
the plastic wrappers of the towel and soap dish in
the bag.
Hand washing prevents possible infection from
care provider to the client
6. Put on apron right side out and wrong side with
crease touching the body, sliding the head into the
neck strap. Neatly tie the straps at the back.
To protect the nurses uniform keeping the crease
creates aesthetic appearance.
7. Put out things most needed for the specific case
eg thermometer, kidney basin, cotton ball, waste
paper bag and place at one corner of the work
area.
To have them readily accessible.
8. Place waste paper bag outside of work area.
To prevent contamination of clean area.
9. Close the bag.
To prevent contamination of bag and contents.
10. Proceed to the specific nursing care or
treatment.
URINE TEST FOR ALBUMIN
1. Cleanse and assembles all articles/equipments
needed.
2. Prepares 3 or more cotton balls with
denatured alcohol or alcohol lamp.
3. Fills the test tube 2/3 of urine. (Note: urine
specimen should have been collected early in
the morning before breakfast).
4. Heats the upper portion only of the test tube
with urine without shaking. Compares the
heated to the unheated portion for changes.
5. Read and interprets result correctly.
URINE TEST FOR ALBUMIN RATIONALE
a. If there is no change on the heated portion,
the result is negative. Acetic acid is not added.
b. If cloudiness appears:
b.1. add 5 drops of acetic acid to the urine.
b.2. heats again to rule out presence of
phosphates.
(Note: if cloudiness disappears, result will still
negative: but if it persist in spite of addition of
acetic acid or even deepens, the result is positive.)
6. Discard things appropriately.
7. Explains correctly and comprehensively to the
client or caregiver the result and
interpretation of the test.
URINE TEST FOR SUGAR
1. Measure 5ml Benedict’s Solution in medicine
glass.
2. Holding the test tube with the holder, heat it
over a spirit lamp till the Benedict’s solution
boils without overflowing.
3. Drop 8-10 drops of urine into the boiling
Benedict’s solution.
4. After again boiling the mixture let it cool
down.
5. While cooling the mixtures changes color.
6. The color of the mixtures serves as a guide to
the amount of sugar in the urine.
7. Read and interprets the result.

8. Discard things used appropriately.
9. Explain correctly and comprehensively to the
client or caregiver the result and
interpretation of the test.
10. After completing nursing nursing care or
treatment, clean and alcoholize the things used.
To protect care giver and prevent spread of
infection to others.
11. Do hand washing again.
To deter the spread of microorganism.
12. Open the bag and put back all articles in their
proper places.
To be ready for the next use.
13. Remove apron folding away from the body,
with soiled side folded inwards, and the clean side
out. Place it in the bag.
14. Fold the linen/plastic lining clean place it in the
bag and close the bag.
15. Make post visit conference on matters relevant
to health care, taking anectodal notes preparatory
to final reporting.
To be used as reference for future visit.
16. Make appointment for the next visit (either
home or clinic), taking note of the purpose.
For ff.-up care
17. Sees to it that the waste receptacle and its
content are properly disposed of before leaving
the home.
18. Maintains body mechanics throughout the
procedure
19. Manifest neatness on the performed
procedure.
20. Provides and ensure patient’s privacy
throughout the procedure.
21. Observes courtesy.
22. Shows calmness while performing the
procedure.
23. Depicts mastery of the procedure.
24. Uses correct grammar and syntax.
