APPOINTMENT OF A GAR9.2 INCIDENT INVESTIGATOR
DUTIES AND RESPONSIBILITIES:
You are to ensure that the Occupational Health & Safety Act 85 of 1993 and the regulations applicable
are complied with in your area of responsibility.
You are hereby appointed as the GAR9.2 Incident Investigator.
Your duties in this regard are summarised as, but not necessarily restricted to, the following:










once you are advised/become aware of an incident you must immediately arrange/start the
investigation process, in accordance with the companies Incident Investigation procedure.
where reasonably practicable you must visit the site as soon as possible, preferably the same day if
possible, with the view to establishing:
The exact time and date of the incident.
the conditions at the time of the incident.
Who witnessed the incident.
Statement of witnesses and all other relevant parties.
Collect all evidence, including photos.
all incidents must be investigated within 7 days of the incident and finalised within 14 days.
the results of the investigation must be recorded and tabled and discussed at the first meeting of
the Health and Safety Committee following the incident.
all documentation pertaining to incident investigations must be properly documented and filed for
storage.
You are required to acknowledge your confirmation of training and your acceptance of the
responsibilities and implications of this appointment.
APPOINTMENT
I, __________________________ (Sec 16.1) of Thuthukani Cleaning and Gardening do hereby appoint you,
_______________________ being a full-time employee, with the duty of a GAR9.2 Incident Investigator for
Transnet Port Terminals Housekeeping project for the duration of the project/contract or until you are
relocated to another site/project or leave the employ of the company.
Your appointment will be valid from the date of acceptance till or such other date as decided and documented
in writing by the Sec 16.1/16.2.
You are reminded that you are required to be conversant with all relevant statutory provisions and regulations
of the Occupational Health & Safety Act 85 of 1993 and the COID Act 130 of 1993.

ACCEPTANCE OF APPOINTMENT
I _______________________________________ hereby accept this appointment as a GAR9.2 Incident
Investigator and confirm that I have the necessary competence required and that I am conversant with all
the relevant statutory provisions of the Occupational Health & Safety Act (85 of 1993) with regard to carrying
out construction work.
