Rate:
□ Normal (60-100 beats per minute (bpm))
□ Bradycardia (<60 bpm)
□ Tachycardia (>100 bpm)
Rhythm:
□ Regular
□ Irregular
P waves:
□ P waves occur before each QRS
□ P waves look the same shape and size (morphology)
□ Absent P waves
P-R interval:
□ Normal (0.12 sec to 0.20 sec)
□ Short (<.12 sec)
□ Long (>.20 sec)
□ Cannot determine
QRS complex
□ QRS complex occurs after each P wave
□ QRS complex looks the same in shape and size (morphology)
□ QRS complex is normal (0.06-0.10 (some may say upper range is 0.12sec)
□ QRS complex is wide( >0.10) or (>0.12 sec)
Q-T interval
□ Normal (<0.52 sec)
(less than half the length of a R-R interval)
