What are the reasons for taking this specific course? The reason for taking Math 1220 again is for me to complete my requirements in order to transfer to SFU this Winter 2024 and to also take classes that requires Math 1220 as a requisite. 2. Rationale as to why you were unable to successfully complete the course in your previous attempts. Please provide details for each attempt. a. Detailed rationale for the 1st attempt: I was unable to successfully complete the first try because I was really having a hard time balancing 2 math courses and a physics course during Winter of 2022. I was continuously in a battle of understanding the concepts of the course in the quizzes and in tutorials. b. Detailed rationale for the additional attempt(s): 2nd try: I was actually doing well in the course and was understanding the concepts slowly. However, I had to contact my professor to reschedule my final for the next semester as I caught COVID during that time and have provided him with proof of a picture of the antigen test. As the following semester came in, I completely forgot about the examination and eventually missed it. Since during that time, I took a semester break (due to personal reasons) and was working full time and eventually completely forgot about the examination that is going to take place in December (Fall 2022 final exam period). 3rd try: This semester was doing fine as well; however, I have missed quizzes that completely damaged my final grade by 1%. I truly did my best, but my best wasn’t enough. I was going through a lot of setbacks and problems mentally that it completely damaged my academics. At the end, I have passed the course, but it was not enough for me to take the following courses that requires Math 1220 as requisite. So I am here requesting again to repeat the course and get a higher grade. What will you do differently to ensure your success in this course? I will continuously ask for help not just through the math lab and the instructor but to my fellow peers as well. I have retaken this course multiple times but still some of the topics still confuses me. I am planning to get out of my comfort zone and ask for additional help outside of school as well, such as tutoring services. 4. What is your alternative academic plan in the event that you are unable to repeat (or successfully complete) the course? Since I’m transferring to another university, that may be my backup plan and just take it there. However, I would like to take Math 1220 here in Douglas, compared to other institutions, Douglas College has a better support system for the students with the help of math lab and the instructors. Since the classes are made out of small groups compared to 100+ students, this is a better environment for me to learn and gain knowledge. 5. Additional information you would like us to consider (optional). Since I have retaken this course 2 times already, I am still willing to take this course and pass it with flying colors. I have struggled in the past due to unforeseen circumstances and personal setbacks. Yet I still didn’t give up and still willing to face it. Please consider this form as I would really like to continue my studies in the future by facing my struggles step by step.
