PROJECT TITLE : PROECT NUTMEG FOR PT2SB AT PENGERANG,JOHOR DARUL TA’ZIM 4.2 Reinforced concrete work for spreader footing, cross over concrete base, ramp and heavy duty slab cover 4.2.1 Base preparation 1) The sub base material shall be hard, durable and clean. It shall be granular material and shall be free from organic material, clay balls, and any other deleterious substances. 2) The materials will be delivered to the site by tipper lorries and spread and leveled by means of a back pusher for the full width of the road. The material shall be filled and compacted to the required thickness as specified in the drawing. 3) Compaction of the sub base layer shall be carried out by using vibrating roller compactor to achieve relative dry density as per specification. 4.2.2 Formwork 1) Plywood & plastic formwork is using for all structure. All the formworks shall comply with approved construction drawing and the given specification. 2) The formwork is to be constructed in such a manner that it must be able to support the weight of the fresh concrete. Deformation must be within permitted tolerance. 3) Tolerance limits for formwork: Variation from the plumb in 2.50m is +/- 5mm. Variation in the thickness of slabs and walls is +/- 6mm. 4) Design and erection of formwork shall take into consideration of impact loads apart from horizontal and vertical pressure during placing of concrete. 5) For cleaning purpose to leave temporary cleaning opening. 6) The forms shall be sufficiently tight to prevent leakage. 7) The forms shall be cleaned and applied with approved release agent (form oil) 4.2.3 Reinforcement Bar 1) Reinforcement can be fabricated into a variety of shapes that can be used to construct member of any configurations. 2) Reinforcement should comply with BS4449. Different type of reinforcement may be used in the same structural member. Generally, minimum yield point of bars shall be as follow: PROJECT TITLE : PROECT NUTMEG FOR PT2SB AT PENGERANG,JOHOR DARUL TA’ZIM R = Mild round bar, fy = 250N/mm2 T = High yield type 2, deformed bar, fy =460N/mm2 3) Reinforcement bars shall be locally bent to avoid interference with other bars. 4) The reinforcement must be at all-time free from mud, oil or other non-metallic coating that adversely affect the bonding capacity. 5) The placement of reinforcement shall be strictly based on the approved construction drawings and shall be secured against displacement and deformation with tie wires. 6) Where parallel reinforcement is placed in two or more layers, the bars in the upper layers shall be placed directly above those in bottom layer. 7) All laps length shall be 40 x Diameter. Laps shall be provided at the locations shown on the drawings. 4.2.4 Concreting work 1) There will be 2 times concreting works. First concreting for base of drain. Second concreting for drain wall. If the drain wall height is more than 2m, there will be 2 times cast. 2) Concrete shall not be placed until mix designs are submitted to and approved by the engineer, concrete grade to be followed as specified in the drawing. 3) Slump shall be 75mm ± 25mm for normal mix. Slump test shall be made in accordance with BS 1881. 4) No concreting shall be placed until the steel reinforcement and formwork has been checked and approved by the Engineer and QC (IPT). 5) To avoid of cold joint concrete shall be continuously pouring until completion. 6) If cold joint begin, bonding agent (Estobond P) shall be used for concrete repair system. All repair areas should be defined by a 10mm saw cut to avoid “feather edging” of the repair mortars. Pre-soak the repair zone with copious amounts of clean water preferably for a minimum of 1 hour prior to application of the Estobond P. Remove excess surface water with a stiff brush. 7) The compressive strength of concrete is determined by crushed cube test. Cube test shall be carried out at 7 and 28 days after casting or as requested by engineer. PROJECT TITLE : PROECT NUTMEG FOR PT2SB AT PENGERANG,JOHOR DARUL TA’ZIM 8) The level will be control by putting the leveling peg and finishing by skill levelling workers. 9) To avoid excessive segregation of the fresh concrete, special precaution should be taken. e.g. extending the hopper by means of a pipe or sock; and the concrete should not be allowed to fall freely for more than about 1.00 meter. 10) Extreme care must be taken as not to alter the location of starter bars which must be properly secured during casting. 11) All concreting shall be thoroughly consolidated during placement and shall be thoroughly worked around the reinforcement fixtures by using a sufficient number of vibrators/ pokers. 4.3 Re-attach existing fence and make good of surface 4.3.1 Mark and manually dig to lay fence post. 4.3.2 Roll back chain-link to middle of fence post. Tie-up and strengthen the chainlink. 4.3.3 Trim and make good of surface gavel surrounding existing fence. 5.0 SAFETY ASPECT 5.1 Safety 5.1.1 Safety induction to be conducted to all new workers that involved to this activity and safety toll box meeting shall be conducted every day to remind the workers about concerning the safety. 5.1.2 All workers shall be provided with PPE (Personal Protective Equipment) and shall be worn at all time on site. Safety Manager / Safety Supervisor shall ensure all the workers wear all the safety equipment’s provided. 5.1.3 All workers shall be advised to report any accident or injury to supervisor. 5.1.4 Proper and adequate provision of temporary signage’s and barricades at diversions and temporary haul road as necessary (if applicable). 5.2 Environment 5.2.1 All workers shall educate on good environmental management practices and well-disciplined to environment. 5.2.2 The garbage bin shall be provided at the working area and Supervisor shall ensure its must be collected regularly and disposed off at the approved dumpsite. 5.2.3 All workers shall ensure that all tools and equipment machinery are in good condition to avoid any oil leakage that could harm to environment. PROJECT TITLE : PROECT NUTMEG FOR PT2SB AT PENGERANG,JOHOR DARUL TA’ZIM 5.2.4 To provide portable toilet for worker 6.0 HAZARD ANALYSIS 6.1 Recording the assessment 6.1.1 6.2 Identifying the Hazard 6.2.1 6.3 Significant finding of the risk assessment must be recorded. All hazards in a workplace need to be identified and necessary precautions taken in order to completely remove or minimize it to an acceptable level. The first step of risk analysis is to identify the particular hazard or group of hazards that require assessment. It should also be decided who the exposed group may be (who might be affected and how). The hazard may have been pointed out someone, it may have been highlighted as a result of a safety audit or observation, and it may be obvious or may apply to a particular activity or aspect. When considering the exposed group, consideration shall be given to others who are not there at the time of assessment. Potential Hazard Event Excavation (Using backhoe &excavator) Transferring wasted material out using tipper truck Potential Hazard - Vehicle collision due to speeding or lose control Prevention - Worker getting hit by machinery - Underground service damaged due to excavation - Assign competent driver with valid driving license. - Banksman to made available -To carry out underground scanning to determine the location of underground services. -For identified gas pipe and cable area, manual excavation shall be carried out to pre-determined depth. - Vehicle collision due to speeding or lose control - Assign competent driver with valid driving license. - Banksman/signalman Backfilling Using backpusher/ bulldozer - Vehicle collision due to speeding or lose control - Worker getting hit by machinery - Assign competent driver with valid driving license. - Banksman/signalman Compacting using roller compactor -Vehicle collide and out of control -Worker getting hit by machinery - Machinery to be inspected and assign with competent driver -Banksman/signalman to be made available PROJECT TITLE : PROECT NUTMEG FOR PT2SB AT PENGERANG,JOHOR DARUL TA’ZIM Night work - Long working hours without rest may cause the workers to be tired and loose concentration. - Lighting - lack of lighting at site may cause accidents. - Adequate rest must be given to the workers by having two or more shift to the work. - Proper and adequate lighting must provided.
