Scenario for Head, Face, and Neck, Including Regional Lymphatics (SBAR Case Analysis)
Kamal, a 36-year-old Indian national working at the University of Hail's IT Department, was
brought to King Khalid Hospital by Red Crescent ambulance following a road traffic accident
on his bicycle. A motorist slammed his bicycle sideways, throwing Kamal off the bik e and
into oncoming traffic. During the collision, his helmet shattered. On examination the initial
diagnosis was traumatic brain injury (TBI), BP is 80/60 mmHg, HR 98 BPM. O2 Saturation
of 86% and GCS is 6 and required intubation before being sent to the Intensive care unit.
The patient had moderate right intracranial bleed with ventricular and peri-mesencephalic
cistern obliteration consistent with a trans-tentorial herniation. Large, depressed fracture
in the right temporoparietal area large scalp wound Right clavicle fracture, 4th rib fracture,
and diffuse brain swelling. According to his wife, the patient had no significant medical
history, but he had ORIF at right radial fracture 5 years ago in Mumbai, India. MRI revealed
a mild subdural hematoma and diffuse axonal injury. The nurse on duty informs the OR as
ordered by the attending physician to contact the operating room for possible STAT Hemicraniotomy for the removal of epidural and subdural hematoma to manage intracranial
pressure. Left craniotomy for the placement of an external ventricular drain, which
remained in the Neuro intensive care unit for 14 days. Tracheostomy tube placement 4 days
following the accident, PEG tube for nutritional support.
