Plan Description
CIGNA Open Access Plus In-Network with HRA: Covered for in-network providers
only. You'll pay 100% of out-of-network care, except for emergencies. No referral required
to see specialists. Includes Rx coverage, a Health Reimbursement Account (HRA) - An
employer-funded account that helps pay for qualified health care expenses. The Open
Access Plus (OAP) network is a group of doctors, hospitals, facilities and labs that have
contracted with your plan, and is located across the United States.
CIGNA: CIGNA DMO: No patient co-payments for preventative care. You are required
to select a dentist from a panel of participating providers to direct your care. CIGNA PPO:
The freedom to choose either a participating dentist or, for a nominal fee, any non-network
dental care provider. Dentists in this network may provide a better coverage level with
greater savings.
VSP: Voluntary vision plan with the largest national provider of eye coverage. Examination
and lenses every 12 months and frames every 24 months.
AFLAC is a supplemental insurance company that allows you to elect benefits on a
voluntary basis such as: short term disability, accident, cancer, hospital, life insurance.
CIGNA: $75K group term policy plus $75K AD&D. NCHC employees also have the
opportunity to elect voluntary life insurance for themselves and eligible dependents.

need including issues involving substance abuse. Group or individual counseling is available
based on a need in a variety of settings including over the telephone, in-person, or as an inpatient. Additional work/life benefits include dependent care referrals, legal and financial
planning assistance.
Empower Retirement: Retirement savings plan permits employees to defer a portion of
salary on a pre-tax basis up to IRS limits. NCHC offers a 3% of your salary safe harbor.
Vesting schedule allows 100% vesting after 5 years of service. Loan provision available.

3 weeks during the next five, and 4 weeks after 10 years of service. Exempt employees earn

New Jersey Earned Sick Leave: You accrue earned sick leave at the rate of 1 hour for
every 30 hours worked, up to a maximum of 40 hours per year. May be taken for personal
illness, medical appointments, or the illness of dependent family members. 40 hours
carryover permitted however no more than 40 hours can be used within one year.
Employee earns one personal day (8 hrs.) in each quarter of the calendar year as determined
by date of hire
