AUTHORIZATION
This will authorize my son, the bearer, _____________________ to encash
and claim the proceeds of any bank cheque issued in my name as payee in my
behalf. Due to some unforeseen circumstances, I will not be present myself.
Therefore, he has my full consent to take the aforementioned action.
