ROLES AND RESPONSIBILITIES 5.1 CONTRACTOR Site Manager • He is the wholly authorized representative of Subcontractor at construction site in dealing with owner, EPC contractor and local government. He is responsible to all the issues related to site construction management, implement contract articles and project management requirement of Owner/EPC contractor. He is fully responsible for the safety, quality, schedule, cost and final results of the project. • He will attend the weekly site meeting organized by EPC Contractor and is responsible to carry out the decided issues. HSE Manager • In relation with Construction Manager issuing and up-dating safety procedure and advising on its application & effectiveness. • Review and comments on HSE documentation produced either in engineering office or at site by Contractors or his Subcontractor. • Assess Subcontractor implementation of safety program against the documentation issued. • Assess competence of Contractor or Subcontractor personnel involved in HSE matters. • Reviews and if necessary assists on Contractor Training program. • Be responsible for ensuring that information obtained about incident /accident is given to Project Director in case of significant incident /accident. • Ensure that incident analysis is thoroughly carried out and suitable corrective measures are defined. • Proposes HSE improvement program. • Implementation of HSE training program • Supervise basic safety training. LIFTING PROCEDURE 8.1 Site preparation 1) Ground improvement Please refer to the Lifting Plan Drawing for the areas of crane standing. The maximum Ground Bearing Pressure (GBP) of CC8800 is 20 t/m2. Density report and third part compaction certificate are required before lay the crane mats. Take the heaviest equipment for example: When the main crane (CC8800 1250ton) is lifting Existing & New Evaporator C-2536, there are 6 pieces of crane mats under each crawler. The dimension of the crane mat is as follow: Mat height: 0.3m Mat length: 5.0m Mat width: 2.8m According to ground pressure calculation report (See attachment), Max. GBP under the mats during the Existing & New Evaporator C-2536 lifting: 19.30 t/ m2. The underground facilities should be certified safe. A protective measure will be taken if the underground facilities can’t meet the lifting requirements. Soil bearing capacity of the existing area where the crane will be positioned during lifting shall be investigated by Third Party Agency and corrective measures will be applied to the satisfaction of WISON. 4) Communication Need to be opposite start with radio if we have any problem with radio we will use hand signal. International hand signals (ASME B30.5) should be used by rigger during the lifting process. In case of crane operator didn’t see the signal clearly, radio shall be used to execute the lifting activity 5) Weather condition Heavy lifting crew shall pay attention to weather forecast and ensure the good weather condition when the equipment to be lifted. When the weather is low visibility, all lifting work shall be stopped. No heavy lifting activity to be executed if the temperature is too high. Work in night should be have adequate lighting. 8.2 Lifting works 1) Lift plan preparation Ensure that all relevant documentation which including critical lift plan, certificates for lifting tackles and other lifting accessories shall be submitted and approved by Contractor & Client prior to equipment lifting activity. And attached copy for all this certificates in work permit for each lift. a. Rigging plan prepared by a certified Rigger 1 and approved by Third Party Certification agency will be submitted and approved for each critical lifts prior to lifting. b. After the crane assembled shall be inspected by authorized third party as well as the certificated to be issued and attached copy for all this certificates in work permit for each lift. All lifting equipment, lifting tackles, and accessories shall be pre-lifting inspected and ensure they are color-coded, certified and valid. And fill in all kinds of checklist before lifting activity. 3) Hazard area setting Hazard area are set with barricade for lifting activity prior to lift, only the authorized persons are allowed to work inside, and other activities and unauthorized persons in this area must be stopped and keep them out. After the equipment erected, the hazard area shall be cancelled or changed. In addition, road closure permit shall be prepared in advance if public road occupied by lifting activity. Pre-lifting shall be done for the existing equipment to check the gravity of the equipment and exact weight of the equipment. 4) Taglines arrangement 2 tag lines will be used as well as they are even distributed of the Equipment bottom. The function of tag lines is to control the movement and adjust the direction of the Equipment. If Necessary will use chain block with belt for final orientation of the Equipment 5) Bundling Removal After rigging, the transportation attachment and lashing material bundling of the Equipment shall be removed and released. All 7) Pre-lifting meeting All lifting crew shall attend the pre-lifting meeting, the lifting engineer, safety and lifting superintendent will declare the technical data and critical factor to the crew with all checklists completed, ensure all related staff 100% understand the whole lifting process. Then all staff shall sign off. Finally, the superintendent will arrange the lifting task to the crew. 8) Clear obstacle Inspect and ensure all obstacle (such as building materials on the ground, foundation. etc. are cleared before lifting. 8.3 Tandem Lifting Procedure Step 1: Command the crane hook above the equipment lugs/Trunnions/lifting cover, the lifting tackle shall be connected the lugs/Trunnions/lifting cover by rigger, if the lugs/trunnions/lifting cover is over 2m height, the manlift or suitable ladders will be used for lifting tackle connection. Step 2: Using CC8800 1250T Crawler Crane with superlift counterweight pick up the Equipment. Step 3: The main crane will lift the lifting the equipment and the tailing crane(If have) lift the tailing part of the equipment slowly, the Rigger will constantly monitor the cranes and the ground condition, at the same time, the Rigger and crane operators also constantly monitor the weight lifted with the cranes to make sure to stay within lifting range. Step 4: The tag lines will be fitted on the bottom and used to enable the control of any swing/oscillation induced by the equipment. The Equipment will be totally hanged by the crane and maintain the working radius and swing the load in designated direction above the foundation. The equipment will be placed onto the foundation in stages as per procedure from the Main Construction Contractor. During this process, the Superintendent will observe the movement of the boom, equipment, and vehicle body to ensure there is no clashes or obstructions between the boom and structure around, etc. In addition , lifting Superintendent and riggers assist equipment installation subcontractor to secure the equipment onto the foundation, during the process the rigger ensure to keep good communication with main crane operator, and when the equipment is going down close to the foundation, people must pay attention to your hands to prevent hands squeezed by the equipment. Step 5: The equipment will be aligned using shim plates and positioned vertical within limits as defined by specifications. The nuts/bolts will be attached and the load is secured. Approximate 0% of the gross weight will be kept for alignment by surveyor. Housekeeping will be taken after the tackle removing. 9. SAFETY REQUIREMENTS 1) The lifting works shall always be carried out in a safe manner, with all site personnel, staff and labour, fully adopting the project safety procedures, Hazard identification and control program. All parties shall be particularly aware of safety requirements concerning working in structural steel erection. 2) Prior to start of activity, a work permit shall be secured in place. 3) The supervisor in-charge for barricade work shall be experienced and knowledgeable to the nature of the work and he must have the ability to assess risks related to the execution of the work. 4) Existing plant utilities and facilities shall be well protected with safety barricades or bollards whenever these are mounted very near to the location of installation of hard barricade work. Safety and traffic signage shall be installed. 5) Other safety policies and procedures laid by the client’s safety department shall be complied at all times. 6) Where not possible to do the work on the ground or part of a building or permanent structure, Scaffold platforms must be provided in order to carry out the work safely. Experienced and qualified Scaffolds are only allowed to erect, dismantle and modify scaffolding under the supervision of certified scaffolding supervisors. 7) Only certified Scaffolding Inspector / Supervisor shall inspect, approved and sign Scaffolding tag prior to the utilization of the erected scaffolding. 10.
